Typically, I find I can take the most when flat on my back. It also allows me to massage my belly to help move the water deeper and has the benefit of being able to watch the bag deflate.
I would like to lie flat on my back. However, my nozzle is too large to manage that posture. I tried it several times lying at the edge of the bed to avoid interference between the nozzle and the bed. But it was not very confortable. As you know, no one takes that kind of position when you sleep. Also I found that it is difficult to get up after filling around 5 quarts. By above reasons, I usually take all fours or knee chest positions.
The largest enema that I can currently take is six quarts. I will take the first four quarts on my hands and knees. Then I will take the last two quarts kneeling down with my body in an upright position. I find that for the last two quarts, if I am in any other position, I can't take it all if I am in any other position.
Left side for a while, then right side, but then on my knees upright for the last 2 quarts of a six quart enema. I find that if I stay lying down for a full six quarts it is a little difficult to stand up. Also, I think the last 2 quarts goes in better in the kneeling position.
During my teen years I explored a variety of positions for large enemas including a handstand with the bag hanging at ceiling height. I no longer use that as I tend to get dizzy in that position now.Later, in the Military, our corpsmen, lacking a surplus of beds and cots would send guys to the shower with a disposable plastic Bagenema and a slip of instructions to add water and take the contents standing up. I got fairly good at taking an enema standing up, sometimes bending over and grasping my ankles to consume the last few pints.Years later I read that a standing position was best for the finale of a large volume enema so I occasionally hang a bag with the last 2 quarts in my shower, awaiting my arrival from the bedroom. It has a bulky plug nozzle on the hose. I proceed to bed and inject the first two or three quarts of solution with a long colon tube along with plenty of massage to concentrate the solution in the transverse colon and cecum. That done I step to the shower, insert the plug nozzle and take the final two quarts. I seem to be more comfortable with this procedure than taking the whole load while horizontal.
Well, I'm really not a fan of large volume enemas. They tend to make me sweaty and nauseous, even when I use an isotonic saline solution. But, the position that aids me to take the largest quantities would have to be on my back, in bed, with my pelvis elevated on a couple of pillows.
Definitely flat on back. Knee chest puts pressure on swelling abdomen, simms position means the water needs to travel up hill when filling transverse colon. Just my opinion/experience...
Don't do large volume very often. But when I do I generally start out on left side then roll to my back.Sometimes when I'm on my back I'll raise my butt to create a downhill flow to the transverse. I may even roll to the right a bit to get the water into the ascending colon.
Four quarts is my limit. I start off with a couple of 2 quart prep enemas, and then get comfortable on the bed, or floor or wherever I'm going to take the 4 quart. That's import. get comfortable. I will usually start by taking the first 2 quarts on my left side, then roll onto my back for the third, and then transition onto my right side for the 4th. This process usually takes about 10 minutes.
I am not into large volume enemas but there are times when I do want the enema to fill high into my colon. The main concept is simply related to using gravity to let the water works it way up into your colon. So here my main two positions to achieve this:1) Ass up and head down. This lets the water drain up your colon and thus more water can be taken in.2) Lay on your left side. This position gets the water over from your rectum to your ascending colon which is now parallel to the floor while lying this position. Once you have taken on what feels like a good amount of the enema you can switch back the ass up and head down position to let the water drain to upper colon.
You can use any position or combination that feels good to you and do try them all(butt up belly down fills fastest i think) its really a 4 foot shop vac hose and you are filling from one end with water under more pressure than inside the colon gravity has little effect so you could even clean out on the space station! all the anatomical moves may increase the rate its filled or slow it down
I have always found that lying on my left side with your knees slightly drawn up (Simms Position) has always been good for me I find that I can take a rather Large Enema nice & soapy & warmRegards Dave J
I have found that a comfortable position for me taking a large volume enema, varies from time to time. While the knee chest position is good for a deep fill, it is not really that comfortable. I like being on my back, hips propped up on a pillow, watching the flow meter spinning as I am slowly filled. If taking it on the floor, I can raise my legs and prop my feet on the bathroom counter for added elevation if needed.
On my side. It doesn't seem to make a difference as far as left or right side. My definition of a huge enema is about 2.5 quarts btw, I just can't do the volumes that some people on here do.
For me I can take the largest volumes when on my back. easier to relax any abdomen muscle that could prevent the necessary expansion.
For awhile I thought on my back was best. Lately I think Sim's is best because you can let your abdomen pouch out and take more liquid. On one's back the weight of the full colon produces an early filling. I wonder if it also compresses the vena cava and causes distress?

YMMVMy favorite way to induce stress taking an enema is suspended by my ankles (or someone else suspended by their ankles. The weight of water presses on the lungs, heart and takes up breathing space, not to mention all the blood flows to your head. It's also fun to spank my husband that way!This must not be done by people with any circulatory, vascular or cardiac problems. You can get a stroke or blow an aneurysm and die!

I always start in the knee-chest position to get things going and flowing, then I lay on my back wit my legs propped up on the edge of the bath tub. In that position I can easily take 4 quarts.
