I am a huge believer in elbows and knees, since I enjoy being milked as I am filled. You can have you stomach massaged for volume, have a larger nozzle introduced and I believe take your maximum volume. I have taken them in the hospital and you must be on your side because lying flat you can't expand. I have taken them in an "enema chair," one that has an open bottom, but knes wide and elbows is the best delivery method.So girls insist on the gyno stirrups but gravity tells us you are far better with your belly botton facing south.
So this is probably a very common sense question, but I am assuming if you are going to go for a "max out" you should do multiple cleanouts before the "big one?"

Once the volume I choose is all in and I'm ready to retain for a good long while I'm almost always lying on my back. Occasionally with a very large volume enema I'll flip over to right side down, especially if I start to feel like there's unequal pressures and the water is collecting towards my left abdomen trying to get out. I did two 4 quart retentions the last couple of evenings and timed them lying on my back. I spent the entire 30-35 minutes on my back with knees up or extended depending on how much internal pressure I wanted to enjoy.After the retention, when I pulled myself up from that position towards upright, I tried staying on my hands and knees, belly down, and sort of slowly crawled over the six feet or so to the commode rather than standing up to walk over. I had leaked a bit once or twice last week standing up to move, but this hands and knees position makes it easier to move without leaking, even with my DBCatheter inflated inside. My belly felt comfortably distended hanging down like that - maybe I'll try the whole retention or at least a good deal of it tonight like that, the way others have done it knee/chest position.With any luck I'll do the same tonight, maybe with a touch more volume, like 4.5 quarts of warm saline. I like on my back for a large volume enema however there are those that like on left side for all enemas...just depends on the mood I'm in.

I just noticed a couple of nights ago when I was retaining a 4+ quart soapy enema that for some reason it's easier for me to get at least a partial erection while retaining on my right side down. It may be because my belly distention moves down to the right and gives me more and easier access to myself for masturbation while holding water. At this age, anything that helps orgasm production is a plus. Most often, I'll enjoy retaining a large enema on my back, but I sometimes lay right side down after it's all run inside.Eric

My largest volume enemas have occurred when I'm standing. My first 5+ liter was a surprise when I emptied a 4.5 liter bag and then added another liter. My first 6+ liter was emptying a bag and then filling it with an additional volume to come in just over 6-liters. Both of those occurred while I was standing (just slightly bent over),I can routinely take 4-4.5 liters on my knees and elbows and just about any other position.

In knee-chest the first quart or two goes in quickly and comfortably, but then with my abdomen down and tilted towards my head things start pressing on my bowel and my bowel starts pressing on things and it gets much less comfortable. If I start knee-chest I'll usually end up either "doggie" with a horizontal torso or in a different position if I'm going for more than about 2 quarts.For a similar but more comfortable position try lying on your back with something under your butt to raise it up 6 inches to a foot (15-30 cm).
Start on the left side, roll to back and then briefly to right side. Then back to left side. When I do that the expulsion will take longer.
The knee-chest position is best for propagating the fluid through the entire colon.The Simms(Left-side) position is best for relaxation.Relaxation is an important key to both taking and retaining almost any enema, and certainly a high-volume one.Propagating the enema through the entire colon is also critical to taking and retaining a large enema.Those are the two positions I use for anything other than a foreplay enema!Sooo, It's hard to say which of the two enema positions is really best. You will have to experiment and see what works best for YOU.I certainly would not thin k you would want to use any sort of standing position or one where there is pressure applied to the lower abdomen!You can also try a balloon-type retention nozzle. I usually start on my left side and once I get my colon tube about a foot or more in I roll over on my back and continue to feet the remaining 4 feet up my colon. I can take 3 to 4 quarts this way, particularly if I've had a fairly good clean out thus far.
For my large volume enemas ... I start out in the Sim's Position ... then I take my next 2 bags in a knee chest position. It works for me.... the knee chest position allows my wife to rub my back and tummy effectively relaxing my body and stomach to allow the additional water to enter.
Knee-chest very fast flow and hard to relax.Prefer start in Sim's position on left side with colon tube, than move on back with rubber bed pan to up bottom.Sometimes start on back with double ballon catheter on bed pan (to simple release after retention).
For the first few liters - Sim's. Later - with the back down and the bottom slightly raised. For convenience, I can stay under its pillow. Only the solution should be injected very slowly - about 15 minutes.
Question in regards to positions and the average time a full 2 quart enema takes from insertion to removing nozzle?
In my experience the best position for comfortable taking 3-4 liter enema is on the back.It makes tummy available for me as a doctor to massage.The additional improvement can be achieved from pillow under the hips. Or - the ultimate variant - the bed with electric motor that allows to raise the legs section.Maybe it even more fun with medical examination chair. But i'm not experienced it as a patient nor as a doctor.
For a nice big enema I like to start out with them in the knee chest position and then towards the end flipping over to the back seems to relieve some pressure. Bringing the knees up can help and the view is nice too!
If I'm struggling to take the amount I want then I'll usually shift positions around several times from my left side to my back, then crouched on all fours letting my stomach distend and lowering my head towards the floor. Sometimes for the last liitle bit I will transition to crouching upright on my knees and toes to watch the bag empty as it hangs beside my face.

Lying on my left side with my left leg straight and my right knee bent close to my head. When half the bag is empty, I switch to lying on my right side until there's about ¼ of liquid left in the bag, then take the remainder of the enema lying on my left side. When the bag is empty, I hold it all in as long as I can while alternating positions, sometimes lying on my back then going into a doggy style position with my butt high. After that, I stand up. This is where I cant hold it in any longer and I have to hurry to sit on the toilet. The expulsions are explosive and very violent, especially if there's a lot of poop inside of me.
Nice topic…I personally love a large volume enema, I only attempt a 4qt enema if completely cleaned out first. My partner [Barb] and I have a kind of unique setup, I've a large bath - I also have a salon chair that lets you recline wayyyy back - we have added leg rests similar to the newer ‘gyno’ chairs. This position is great for taking lg enemas. On occasion I'll also take one standing. Almost always a lg enema is taken slowly using a colon tube.
