Well if you want the "bending over" to be particularly humiliating, have the patient stand on something that will elevate their waist well above the level of the exam table.
The doctor used to just want me to stand on the floor and bend over the exam table at a 90 degree angle, which isn't too bad. But the last time I had it done, she told me that you are really supposed to stand on the little "step" that slides out of the exam table. The effect of this is that you end up bending over about as far as your spine will let you.
Basically, the further you bend over the more the prostate is pushed against the rectal wall making it easier for the doctor to feel your prostate. This is why my doctor favors this position.
You wouldn't think that a few more degrees to the angle (More like a 'V' shape than an 'L' shape) would do so much to increase the humiliation, but that definitely did it for me. I started to bend over (me, who has no shame) and suddenly found myself too embarrassed to continue and asked her if it was really necessary for me to bend over so far?
She smiled and giggled a bit and said "Well, yes, it really is the best way to do the examination"
Her smiling made it even more embarrassing for me. I mean this is my REAL LIFE doctor. Not some kind of play exam or something...
