The space between the two balloons should be about 1/2 to 3/4 inch at the very most.
My husband and I have had over 15 double balloon rectal catheters over the last 20 years, and still have about ten in different sizes (250/250, 500/250 and 750/250) in latex and silicone. About ten have been silicone.
The latex ones have all had the balloons less than 3/4" apart, and some have been a little more than 1/2 inch. On the silicone ones, the balloons have been as far as one inch apart, and that is way too far. A couple of the silicone ones have had the balloons about 1/2 inch apart.
The ones my husband and I like have the balloons 1/2 to 3/4 inch apart. The problem with the balloons being too far apart, is that the balloons slide in and out a little while you try to retain the enema, and because it slides, the seal is lost, and you leak. The purpose of the outer balloon is to keep the inner one pulled against the anal ring, and when the balloons are too far apart, the inner balloon is pulled inward and the seal against the anus is lost, and again, you leak.
Therefore, as I mentioned above, The space between the two balloons should be about 1/2 to 3/4 inch at the very most.
