The only time I had a pre-employment physical that wasn't just a urine drug screen was back in the late 80's at a pharmaceutical company that had its own Health Services dept. which was then equipped with its own receptionist, 2 nurses, and a male doctor that was on site part time. For the next 20-something years after that, they gave each of their employees an annual physical. The only difference between the pre-employment physical and the annuals was the urine screen, aside from various changes over time.
This physical was as thorough of a physical as I had ever had. The nurse conducted: blood pressure and sample, height & weight, in depth eye exam, hearing, blowing into a vacuum cleaner hose and watching the graph of my exhale on a 1980's technology DOS computer screen, and I forget what else
After all the preliminaries, I was taken into the doctor's office, which had its own adjacent toilet where patients would be directed to strip down to their undies (and give the urine sample too). The room itself had an exam table in the center, and the doc's desk against the wall. I forget what color briefs I wore that day (maroon or dark blue?), but that was all I was wearing as I was told to have a seat on the exam table. Though I didn't get into medfet right away, this physical was where the onset of it began.
After a brief HEENT check and listen to my chest and back with the stethoscope, the doctor asked me to lie back on the exam table, as he pulled out a flat metal extension so that I could rest my lower legs. Just as he would do in subsequent annual physicals, he gradually auscultated and palpated his way down my body, until suddenly, he grabbed my briefs and lowered them, thereby exposing my genitals. I found it a bit shocking, as until then, I had always been standing when told to “drop (my) pants”, which i would do myself.
So while lying on the table, I had no idea I was going to be checked for hernia that way, let alone that when the time came without any verbal indication, the doctor would just go ahead and lower my briefs on his own. And at the time, I still wasn't really used to being exposed in front of anyone.
After the hernia check where I was told to cough a few times, that essentially concluded the exam, with the doctor pulling back up my briefs and telling me I could get dressed, as he went over to the sink and washed his hands. It wasn't until a few years later that there was a raised awareness of testicular cancer, with the nurse having me watch a training video on doing a self exam of them. Although the doctor proceeded to check them himself after the hernia exam. again without any verbal forewarning that he would be gently squeezing my balls. That helped spark the eventual development of my medfet, which didn't really start until several years ago when my homophobia went away, and I finally became okay with men touching me.
Since 2019, I've been hired 4 times by pharma companies, and all that was required was a pre employment drug screen. Not even asking / requiring a clean bill of health from my doctor.
