I, too, prefer the term “torment” over “torture”. While I think both can be used, the term torture gives me more of a forced/involuntary sense. If you’re simulating CnC, and going hardcore for someone who loves her pain, by all means.
Personally, I prefer to be the stern, but caring doctor who does what is necessary, yet uses his own strength to get his patient through her painful ordeal. Lately, I have to say I credit Doctor Ray from @nickle1 ’s recent story. He’s a perfect caricature of a doctor/Dom in a single person.
I think the most enjoyable session would involve phrases like:
“I am very sorry that we need to do this, it will be painful but it’s the best thing for your health. Can you be strong for me?”
“Be still. It’s okay to cry out, but if you move you could hurt yourself, and I won’t have that, understood?”
“I understand how painful this, just a few more [twists/injections/minutes] and it will all be over. Now, be a brave girl for me…”
“Yes, the needles will be unpleasant. Yes, you’ll hate me and all of my offspring while I’m injecting you. But no, I won’t skip this because it’s the best way to help you feel better and stay healthy.”
“Hang in there, I’ve got you - I know it hurts, but I know you’re strong enough to get through it, just listen to my voice and it will all be done before you realize it.”
See, there’s just this beautiful place where the caring nature of the dominant turns into the knowing and capable nature of the doctor. That’s the place where I think labial/clitoral/vaginal torments in medfet and play exams is perfect. Show me in your eyes that you trust me to hurt you, to take care of you, and to trust that I know just how much you can take and not to push you past that point.
