I feel like there would be something fun/interesting about surprise exam but realistically I would probably want to know beforehand.
I went to a new doc to discuss my severe migraines and ended up getting a thorough physical. That was 30 years ago. I would not have cooperated now. If I want a physical I will schedule one. So no surprises for me.
If i went to my doctor and he said i would like to do a full physical on you now,it would surprise me,but i wouldnt be saying,'oh doc you cant do it now as i havnt prepared myself mentally for it,i would tell him do what you have got to do.

I think usually I would rather expect the exam. I guess if I was at the doctor's office for something else and the doctor said they wanted to do an exam I would go along with it.
No, I wouldn't prefer it and would definitely ask questions as to why it was necessary and what they were looking for. I have a great relationship and confidence in my PCP and it wouldn’t be an issue, but I would still ask. For a new doc, I would dig a little deeper, but I most likely wouldn't refuse it if the answers seemed logical.In today's medical environment, I think a surprise exam would be unlikely. but there could be exceptions.
I would rather expect it so I can know to wear a jockstrap, and preferably one I picked out. I would enjoy certain surprises, as a sudden lifting / lowering / sliding down of my undergarment for a hernia / testicle check, etc
I was seeing a doctor to discuss migraine headache treatment options. He listened to my headache concerns and offered treatment options. Then he said he wanted to listen to my heart and lungs. Well, one thing led to another and I ended up bent over the table getting a DRE. I was way too compliant back in those days. What I should have done was let him listen to my heart and lungs then told him to stop there. Physicals are a waste of time and money.
Seems to me the only reason to do a DRE when treating migraines would be if the doc could reach into the brain and massage the part that was causing the migraine to provide relief. Doubt any doc has a finger that long! 😄. Sounds excessive to me.Perhaps the doc saw that you hadn't had a physical for a long time and used your visit as an excuse to perform a physical. You would have every right to question and refuse that as it doesn't seem at all related to your migraines. Did he evaluate your history of headaches, frequency, triggers, stress, family history etc.? My wife had a sudden onset of sharp headaches (not migraines)and didn't see a doc. Woke up one morning with a stroke - burst aneurysm. Only way to diagnose that would be a four point cranial angiogram, which likely wouldn't be justified unless you had a history.Not a doc here - just speculating.
@Rigel at the time I had a PPO. A coworker told me about this doc. Prior to that time I had Kaiser. Kaiser never did any more than necessary and never did anything special when establishing with a new Primary. But I wasn’t establishing with this guy. I just wanted his opinion about my headaches.Migraines were nothing new to me. I have suffered them since I was 12. That and neck problems. So a complete work up for migraines seemed over the top. Yet I complied. Being new to non-Kaiser healthcare I hadn’t expected it. Never again.
There have been a handful of times when I went to the doctor growing up for a cold or a refill on an inhaler, sitting/lying on the exam table, only for the doctor to mention something about doing a well check while in there, casually checking me head to toe.Definitely caught me off guard each time.
Note sure which one I'd prefer.A schedule exam would make me fantasy about it before. It could become true, or not. But there would be expectations.A surprise one would be stressful at the moment, caughting me off-guard. But it could be a very good memory afterwards, helping me with some masturbation sessions.
