When Angie’s husband called me about her problem and his solution, I was reluctant. Performing procedures on restrained patients is something I only do under specific, controlled circumstances. However, his concern for the hemorrhoid he saw on Angie’s little rosebud and his creative idea made me want to give it a try. Plus, apparently the marital agreement they signed gives him complete control over her medical care and treatment.

Angie’s fear of doctors and needles made her completely non-compliant whenever her husband insisted on her getting it checked out. Thus, she was extremely surprised - to say the least! - when her husband bound her up for a fun little bdsm session and then I walked in!

At first she was just red as a beet and so very embarrassed to be caught totally naked, spread eagle on her tummy. This didn’t get any better for her as I explained I just needed to check out her tushy, especially her little pink anus. This seemed to trigger both a strong sense of humiliation as well as fear of what I would find. In fact it’s fair that for a good minute, she entered a “fight or flight” type of reaction. Unfortunately, in her position she could neither “fight” nor “fly”. Eventually she stopped her extreme movements and gave in to the inevitable.

Her husband helped by holding her buttocks even further apart for me as I spent a good 2-3 minutes simply looking at and gently prodding her bottom and marking up her hemorrhoid, as I had decided that it needed to go. She definitely resisted my finger when I did the digital rectal exam, but her husband told me that they engage in anal play quite frequently so I believe she was bothered more by the fact that it was an exam, and a stranger (we had never met), than merely the pain of inserting my finger.

I assured her husband that I found no internal hemorrhoids or other concerns inside her rectum but that the external hemorrhoid would need to be removed immediately. At that point Angie began sobbing and begging us to release her and that she would go willingly to the office, but I told him to stay strong and to let me take care of her right now so she wouldn’t have to worry about it after today. I assured him that I would use plenty of anesthetic and that she wouldn’t feel any pain whatsoever from the procedure.

“What about the …. You know, the … numbing?”

“Oh, the injections of the anesthetic? Well, she’s not going to like those very much. But it only takes a minute or two, and there should only be 5-6 pokes with the needle.”

Apparently Angie could hear everything I was saying because she went into “fight or flight” once again. I had him straddle her lower back and hold her knees down with his arms and upper body while I anesthetized her.

It’s always interesting to see how individual react to injections in the anal region. Some people are totally stoic, as if nothing was happening at all. Not many, mind you, but some. Many others try to be strong, but when the needle and acidic lidocaine solution hits that area so very rich in nerve endings, they lose their composure and cry and complain like you wouldn’t believe. And then there’s the rest, like Angie, who have suffered from these before or simply have an intuition that it is going to be unpleasant. They start out crying and begging, but when the needle first lances that most tender of tender places, they scream but at this high pitch that you didn’t even imagine. More like a squeal, really, if it will help you imagine it.

Those are my favorite.

Of course there are some excellent techniques doctors can use these days to reduce the pain of injection, but what can I say? I guess I’m just old fashioned!

Anyway, I took care of poor Angie’s hemorrhoid, and her husband was so happy with the results that he’s transferred all of Angie’s medical care to me. She doesn’t seem to like me very much, but she’s definitely one of my most compliant patients now.

So, you see, everything has turned out for the best!
