A Lone Star State of Mind

Now, this isn’t an image from my office, but it’s very, very similar to a setup we use for prolonged vulvar evaluations and educational demonstrations. We actually use something called a lone star retractor.

It has a plastic ring just like in the picture above, and slots all around where you can attach elastic bands that will stretch to the part of the vulva you wish to retract. The main difference, as you can clearly see in the image, is that metal hooks are used rather than simple clothespins. The initial setup does lead to rather more squealing than the above, but once the hooks are inserted into the labia you can be sure they won’t slip or release until you’re ready for them.

Despite its surgical origins, I’ve found it very helpful for a variety of in-office procedures. For example, when doing clitoral sensitivity studies it keeps everything wide open and, with an additional hook placed at 12 o’clock to retract the hood, the clitoris can be stimulated with a wide variety of tools without worry that sudden moves by the patient could cause it to become covered or unreachable even temporarily. There’s also a couple procedures that I am pioneering st the moment - you’ll need to wait until I’m fully prepared to give a complete overview - called Emergency Hymenal Reconstruction (or Hymenoplasty), and Hymenal Cerclage.

But I digress. Most recently I had the opportunity to instruct a small group of aspiring amateur gynecologists on female reproductive anatomy. My patient was very reluctant to allow them to observe her exam, but fortunately her caregiver was most generous. Not only did he allow the students to observe, but each of them were also able to attempt to insert a urinary catheter. They were all a bit clumsy, and need to be a bit … gentler … in their technique, but it was a most beneficial educational experience for them.

Every time that patient comes back in for some work ordered by her caregiver or another exam, I always remind her of the experience and reiterate just how generous and kind it was for her to help educate these fine young gentleman. I can tell she, too, understands the benefits. She cried tears of joy at the memory each and every time.
