Arielle watches in nervous anticipation as she sees Sarah dip the index and middle fingers of her still gloved right hand into the large dollop of lube on the paper towel on the Mayo stand. She feels those two fingers begin their penetration of her tight vagina, as Sarah pushes into her deeper, Arielle lifts her hips slightly and curls her shiny nylon covered toes. Once Sarah has her fingers in as deep as they can go, she begins rotating them 360 degrees, palpating every square inch of Arielle’s vaginal walls. Sarah says “Okay, now I will palpate your uterus and ovaries next.” Arielle feels Sarah’s fingers push up against her cervix, making her uterus move closer to her abdominal wall. She feels Sarah’s ungloved left hand begin pushing firmly down on her lower abdomen, right above her buttery smooth, bare mons pubis. Arielle curls her toes again and flexes her glossy, nylon covered, chiseled calves as she continues to stare at the ceiling, trying to think of anything other than what she is enduring right now. Sarah asks “Are you in pain?” Arielle replies “Not sharp pain, but definitely VERY uncomfortable.” Sarah says “I’m sorry I have to press so firmly, but the ovaries can be difficult to feel otherwise. Almost done with the internal exam, just your left ovary now.” Arielle feels Sarah shift her left hand and internal fingers to her lower left side, and begin the firm palpation of her left ovary, once again causing Arielle to flinch and draw in a sharp breath. Sarah asks “Same discomfort as your right ovary, or worse?” Arielle replies “The same nasty discomfort.” Sarah says “I can sympathize, I hate this part too. All done with the internal exam, your uterus and ovaries are nice and healthy, all feels normal.” Arielle feels Sarah’s fingers slowly pull out of her well dilated vagina, breathing a sigh of relief once her fingers are out. Just as she was relaxing, thinking her embarrassing and humiliating exam was over after watching Sarah remove and toss out the soiled glove, her heart starts racing again as she sees Sarah don a fresh glove on her right hand with a loud snap.

I am going to give you what is called a rectovaginal exam. I am going to insert my middle finger into your rectum and my index finger into your vagina. The reason I am performing this exam is because your uterus is tipped backwards, which is called a retroverted uterus. A retroverted uterus is just a different variation of normal. Many women have either a retroverted uterus like you do, or an anteverted uterus, which is tilted forward. It is quite common actually. The only way I can fully palpate and assess a retroverted uterus is to perform a rectovaginal exam, and use the rectal finger to push up and palpate the posterior side of your uterus through your rectal wall. I will also be able to palpate the space between your rectal and vaginal walls for any nodularity or masses, and also check your uterosacral ligaments. I will follow up the rectovaginal exam with a brief rectal exam, so I can check for any abnormalities in your rectum or rectal bleeding.” Arielle says “Damn, and I thought the bimanual exam sucked. How uncomfortable will this be?” Sarah replies “You will definitely feel a lot of pressure, and you might feel like you need to move your bowels, but don’t worry, you won’t. The more you can relax your bottom, vaginal muscles and abdominal muscles, the less pressure you’ll feel. Remember, if you feel any pain, which you shouldn’t, let me know right away.” Arielle goes back to staring at the ceiling again, trying to “zone out” once again, her heart is beating so hard her head is pounding, and the sour feeling in the pit of her stomach returns.
