Sarah raises the head end of the exam table to about 20 degrees or so, then moves in front of Arielle, reaches down by Arielle’s legs, and pulls the stirrups out from their stowed position under the exam table. She unfolds them to the open position, the stirrups making an ominous click as she does. She then adjusts them wide, the stirrups making a lot of metallic clanging noises in the process, sending a chill down Arielle’s spine. Sarah briefly explains how a pelvic exam and Pap test are done, then she asks “Do you have any questions about what to expect during the pelvic exam?” Arielle replies “Umm, no, between your explanation and what I have read before, I think I pretty much know what to expect, but I am still not looking forward to it, I’m still nervous about how it’s going to feel.” Sarah says “You may feel some mild discomfort or cramping, but no pain. If you do feel pain, let me know right away.” Arielle replies “I will, I guess I am as ready as I will ever be.” Sarah says “Okay Arielle, sit near the edge of the exam table, lie back and place your feet in the stirrups.” Arielle lifts herself and moves her bottom to the edge of the exam table, and as she lies back and moves her feet towards the stirrups, Sarah places her hands on Arielle’s nylon covered ankles and gently helps guide her heels onto the stirrups. Lots of loud table paper crinkling as she gets into position. Sarah says “Now scoot your bottom towards me, a little more, good. Now let your knees fall open as far to the sides as you can.” Sarah holds her hands out wide, to show Arielle how far open she wants her knees. Arielle’s knees touch Sarah’s hands, and Sarah says “Now try and relax as best you can, take slow, deep breaths and try not to tense your abdominal muscles.” Arielle replies “I will try, it would be great if I could fall asleep right now.” Sarah laughs and says “I think all of us women wish we could sleep through a pelvic exam.”

Sarah pushes up the blue paper drape to Arielle’s navel, completely exposing her from her navel down. She then switches on a very bright LED lamp that is attached to a wall mounted swivel bracket, and swings it in between Arielle’s outstretched legs. Adam is mesmerized. It is like Arielle is on display for him. Her pancake flat, firm abdomen, round, tight as a drum ass, and firm, curvy hips illuminated by the bright lights. Her completely bare and stubble free mons, vulva and perianal areas, smooth as butter, the laser hair removal she has had done keeping her that way. He sees her puffy labia majora, her light pink labia minora are small and are barely visible, hidden by her outer labia. Her glossy, suntan nylon/spandex thigh top stockings making her well toned thighs, chiseled calves, and size 7 feet almost look oiled up, due to the very bright LED lamp and overhead lights, and the bright sunlight shining through the opaque window shade. Her entire body from her navel down to her toes is aglow from all the lighting. Adam is rock hard and is eagerly awaiting the exam to begin.

Arielle is lying there, legs spread wide apart, feeling like her ass is about to fall off the end of the exam table, and her heart feels like it is about to beat right out of her chest. She is seconds away from being probed medically in her most intimate area for the first time in her life, and she feels so vulnerable, exposed and embarrassed. She cringes when she hears the snap of Sarah donning a pair of white nitrile gloves. Sarah has a seat on the rolling stool and rolls up in between Arielle’s wide open stocking clad legs. Sarah says “Okay Arielle, I will begin with the external exam first. You will feel me touching you now.” Arielle flinches ever so slightly as Sarah touches the back of her right hand to Arielle’s nylon covered inner thigh. She then feels Sarah’s fingers lightly touching her outer labia, and she begins palpating her outer labia. She then gently parts her outer labia to expose her healthy pink, small inner labia. Sarah runs her fingers along the soft folds, palpating her from near her clit hood down to the bottom of her slit. She then gently pushes up Arielle’s clit hood to expose her clitoris, making Arielle shudder slightly at the sensation. She has never had her clit hood pushed up during an external exam at her previous physicals. Seeing no adhesions or abnormalities, Sarah pushes Arielle’s clit hood back down. Sarah says “All good so far, now you will feel me checking your Bartholin’s and Skene’s glands.” Arielle feels Sarah place her index finger just inside her vaginal entrance, then gently palpate her with the index finger and thumb at the 4 o’clock and 8 o’clock positions. She then feels Sarah’s index finger go slightly more into her vagina and palpate the upper part of her vagina. Sarah says “All good, now I will check you for urinary incontinence.” She feels Sarah’s finger slide deeper and push up, and as she slowly slides her finger back out, she holds her labia apart with her other hand to visualize her urethra and says “Cough hard.” Arielle coughs hard. Sarah slides her finger along Arielle’s urethra through her upper vaginal wall again and says “Cough again.” Arielle gives another hard cough. Sarah says “Very good, no urinary incontinence. Now you will feel my finger go deeper to check the angle of your cervix for the speculum insertion.” Arielle feels Sarah’s finger slide deep into her, then she feels her touching her cervix. Sarah removes her finger and reaches for the speculum. She picks it up by the handle, puts the battery on the Mayo stand, and pushes down to finish inserting it into the handle, which activates the little LED light on the end near the blades. The blades of the speculum light up bright. Adam watches with interest as Sarah lightly lubes up the bills with a thin coat of lube from the large dollop on the paper towel. Arielle sees this and dreads what’s next, her nerves almost making her shake. Sarah says “Next is the speculum exam. This is a medium Pedersen speculum, this is the most common speculum I use on young, sexually active women that haven’t given birth. A larger speculum is usually used on women that are older and have given birth. You might feel some pressure as I insert it, remember to take slow, deep breaths. You will feel me touching you now.”

“Here we go, this is it, the moment I have been dreading since I knew what a speculum is for” Arielle thinks to herself. She feels Sarah’s fingers gently part her labia, then cool, hard plastic just touching her introitus. Then the cool, hard plastic begins pushing into her, slowly going deeper and filling her up, she begins to feel some pressure. She feels the blades rotate and Sarah says “Now I am going to open the blades, you might feel some more pressure and maybe some mild cramping.” Arielle feels the blades begin to open her up, she hears a loud click, and as she is opened wider, more loud clicks echo around the exam room. She feels a lot of pressure and feels more stretched open than she ever has before. Finally, after the fourth click, Sarah stops. Arielle feels some pressure and discomfort, but so far no pain. Sarah says “Now I will be taking some samples for the Pap test and HPV test. The first one will be the worst, I need to insert this little bristle brush into your cervical os for the endocervical sample. You will probably feel some scraping or a slight pinch, and maybe a quick, sharp cramp, but it is over quickly.” Arielle sees Sarah moving the long handled bristle brush down towards the wide open speculum, and it disappears inside her. Suddenly she feels a sharp pinch, then a scraping sensation as Sarah rotates the brush 360 degrees, causing a quick but unpleasant cramp. Arielle’s glossy nylon covered toes curl up tight, her well toned thighs and chiseled calves tense up and flex, and her hips lift slightly. Sarah removes the brush and says “Okay, you did well, that was the worst part, the next samples you will hardly feel.” Arielle finally exhales after holding her breath while having her cervical canal scraped by a stiff bristle brush. Sarah swishes the brush in the liquid filled cup several times, then takes the long handled wooden spatula looking thing, and says “Next is a scrape of your outer cervical opening, you should only feel a slight scraping sensation, not as bad as the last one.” Arielle feels the hard wooden spatula press against her cervix and rotate 360 degrees, the scraping sensation is definitely not as bad as the last one. Sarah swishes the spatula end in the liquid filled cup several times, then picks up the long handled fat Q tip and says “You probably won’t feel this one, this is the last sample I will take.” Arielle really doesn’t feel much as Sarah swabs inside her vagina, in the vaginal pool at the base of her cervix. Adam is fascinated by Arielle’s vaginal canal being opened wide and brightly illuminated by the speculum, and watching Sarah taking the Pap and HPV samples from deep inside Arielle. Arielle makes eye contact with Adam, she sees how intently he is watching her exam, and she knows he is very turned on, much like she was when she was watching him being examined. He smiles at her and flashes a sympathetic look. She smiles back then looks back up at the ceiling. Well, she is certainly NOT turned on now!

Sarah says “Okay, all done with the Pap and HPV samples, now I will remove the speculum.” Arielle feels the pressure and fullness subside as the blades slowly close partially, almost closed, then the blades slowly slide out of her, much to her relief. Sarah sets the soiled speculum down on the Mayo stand after pushing the release tab to let the battery slide out partially to turn off the little LED light. Sarah rolls back and stands up, removes her left hand glove, tosses it into the wastebasket, then uses her left hand to shut off the bright LED lamp, then swings it back towards the wall, out of the way. She uses her left hand to screw the cap onto the liquid filled sample cup, then says “Okay Arielle, you did well during the speculum exam and Pap test, your cervix and vaginal canal looked nice and healthy. Now I will be giving you an internal bimanual exam to check your cervix, uterus and ovaries. You may feel some pressure and maybe some mild cramping, but no pain. If you feel any pain, let me know right away.” Arielle says “Okay” as she feels her face flush and her heart pounding in her head. Adam just keeps looking at Arielle’s glossy, stocking clad, firm, well toned legs, pancake flat, firm abdomen and bare, buttery smooth mons and vulva, knowing that Sarah is about to insert her fingers deep into his girlfriend’s vagina……
