It was about 9:00 a.m. on Monday morning when the telephone rang.

Shirley answered and it was Dan. He said Shirley I need a favor. What

can I do for you asked Shirley. I have a young lady named Ellen in my

office. She is 19, a college sophomore, and engaged to be married in a

couple of months. She came in for a pre-martial exam and after my nurse

checked her vitals, she handed Ellen a gown and instructed her to undress

completely and put on the gown. Ellen said no, why can he just listen to

my heart and lungs over my blouse like my old doctor. My nurse asked

her if she knew what a pre-martial exam was and she said she just

assumed it was checking her eyes, ears, throat, heart and lungs. My nurse

said no, it is a total physical exam including a breast and pelvic exam.

When my nurse told her what a pelvic exam is, she said no, I can’t do that.

She said had never been nude in front of a man before and she just

couldn’t do it. So I am wondering if you will meet with her, talk to her, and

if she is willing do a complete physical exam on her, I can’t imagine why

she is so shy but you can find out. Shirley said of course I will, send her

over.

Shirley was wearing one of her sun dresses, no bra and no panties. She

knew she couldn’t do the exam dressed like this, so she went to her room,

took off the dress, and looked for something she could wear. As she was

standing there nude, she looked in the full length mirror. She liked what

appointment for a pre-martial exam and I did with Dr. Novak. This morning

when I got there there nurse told me to take off all my clothes and put on a

gown and the doctor would be in to examine me. I was shocked. She

doctor has just examined me over my clothes, listening to my heart and

lungs. Shirley said why was it your dad told you to get a premarital exam.

Why not your mother. My mother died when I was 10 and my dad has

raised me alone since then. So when you went through puberty it was

only your dad to help you ask Shirley. Yes said Ellen, it was hard

sometimes. Did your dad provide any sex education. No said Ellen. He

just told me not to have sex until I get married. Did he explain what sex is.

He only told me not to get undressed with a boy and not to let a boy put

really said Ellen. OK said Shirley. I am going to help you. I can’t imagine

what you have been through. If you want me to I will be your friend.

Well let’s start by me explaining to you want a pre-martial exam is. OK

said Ellen. Well it is a total physical which includes a breast exam and an

exam of your vulva, vagina, and rectum. You mean the doctor is going to

look at all those areas. Yes said Shirley. That is why you have to be nude

under the gown. Why does he look at my privates ask Ellen. He examines

your breasts to make sure you don’t have any tumors that can be

cancerous. He must carefully feel every part of your breasts to be sure

there are no lumps, etc. To do that he will do it three ways. First he will

have you sitting on the exam table with your arms behind your neck. Then

he will do it with you laying on your back. The third way is with you on

your hands and knees with your breasts hanging down. He will also gently

squeeze your nipples to make sure there is no discharge, which could

indicate an infection. I don’t know if I could let a man touch me like that

said Ellen. Well said Shirley, it is strictly a medical procedure and women

go through it every day. I still don’t know if I could said Ellen.

Shirley said the exam of your vulva and vagina is to be sure there is no

infection or lesions, and that you hymen is normal. Do you know what

your hymen is? No said Ellen. Shirley said your hymen is a thin area of

skin that partially blocks the opening to the vagina. It normally has a small

opening in the middle which will be broken the first time you have sex.

Some girls unknowably break their hymen doing athletic activities as they

are growing up. But in some girls it can be so thick that it can’t possibly

be broken during sex. In those girls it has to be surgically cut. So the

vaginal exam is partly to make sure you and your husband will be able

able to have sex successfully on your honeymoon. You do want to enjoy

sex with your husband don’t you ask Shirley. Well I guess said Ellen.

Have you can Saml talked about having sex when you get married. Not

really said Ellen. Shirley said a married couple having sex is the most

beautiful and pleasurable experience a human being can have. So you

and Sam do need to talk about it but lets put that off for now. Back to the

pelvic exam.

In addition to checking your hymen the doctor will check the internal walls

of your vagina and check your cervix. He will do a test called a pap smear

to be sure you don’t have cervical cancer. He will also examine your

rectum to be sure the wall between it and your vagina is normal. To do

this he will insert a tool called a speculum and spread your vagina open so

he can see inside it. It doesn’t hurt but you will feel pressure while he does

it. Again I don’t think I could let a man do that said Ellen. Shirley said well

let’s go into the medical room and let me show you some things. Don’t

worry, I am not going to ask you to undress and I am not going to examine

you. We are just going to talk. OK said Ellen.

Shirley led her into the medical room, turned on the lights, and closed the

door. Ellen said you have an exam table just like Dr. Novak. Why do you

have it? Well first of all I do exams for Dr. Novak for young ladies like you.

And secondly I used it for sex education sessions with couples that are

about to be married. But let me show you a speculum. Shirley opened a

drawer and took out a small size Peterson speculum. It was still sealed in

plastic but Shirley opened it. She told Ellen this is what is used to examine

your vagina. After it has been lubricated it will be gently inserted into your

vagina and then opened. Here hold it, open it, see what it is like. It is so

big, said Ellen. I don’t think it will fit in me. Shirley said it is the smallest

size and it will fit inside you. After your vagina and the speculum are

Yes said Shirley. I just don’t know about that said Ellen. Shirley said let’s

put it down and Shirley took out a small Collins speculum. She showed it

to Ellen and said this is a different kind of speculum, it opens the vagina

horizontally instead of vertically. She handed it to Ellen and told her to

open it. As Ellen did she says it opens a long way also. Yes said Shirley,

but again it will be lubricated and it should not hurt. Shirley said I am

showing these to you just so you will know what to expect when you are

ready for a pre-martial exam. Do you have any questions about them. No

said Ellen. OK said Shirley. Why don’t you sit on the exam table. Ellen

climbed up and sat on the exam table. Shirley pulled out the stirrups and

told Ellen what they are and what they are for. She said Ellen why don’t

you lay back on the exam table and put your feet in the stirrups just so you

will know what it feels like. Well if I do you will see my panties said Ellen.

Shirley said I know but I am a woman just like you and I am a nurse. I

have seen panties before. Well OK said Ellen. So she laid back on the

table and Shirley helped her put her feet in the strips. Shirley said now

exam for you. No said Ellen, I can’t. OK said Shirley let’s go have another

cup of tea. Ellen got off the table and followed Shirley into the kitchen and

soon they were drinking tea again. Shirley said Ellen I want to be your

be easy to make, and it could sit on the stove until they were ready for

dinner. She bought the ingredients and also a couple bottles of red wine.

She headed home with the groceries.

She put up the groceries and had lunch. After lunch she took a nap

because she wanted to be rested for Dan. She had big plans for tonight.

chili. She had the table set, the crackers out and on the table, and the chili

naked from the waste up. She walked over to the exam table and stood in

front of Ellen. Ellen was not looking at her so Shirley said Ellen, I want you

to look at me. Look at my breasts closely. Ellen turned and studied

Shirley’s breasts. Ellen said Shirley I think your breasts are beautiful.

Shirley said thank you. But lets start, first with the parts of the breasts.

The tips are the nipples. The darker portion around the nipples are the

areola and the remainder is the main part of the breast. The nipples are

the most sensitive. The nipples are where a baby can suck and get milk

after you deliver a baby. The breasts have two purposes. First to provide

food for a baby when you have one. Second to provide pleasure for you

and Sam. You already told me Saml likes to caress them and he will like it

more when you are nude and he is touching your bare breasts. But now

for the exam. The purpose of the exam is to see if there are any lumps or

tumors. So I start with the tips of my fingers on the outside of the breast

and move in circles gently pushing on the skin to feel what is underneath.

breasts in a circle, and then repeat in a smaller circle, and continue

repeating until I am at the center. When I finish the breast exam I gently

squeeze the nipple to see if there is any discharge. A discharge could

indicate an infection and if there is a discharge I will get it on a slide to

send to a pathologist. Normally during the sitting up portion of the exam

my arms would be behind my neck like this, but I can’t do my own exam

and have my arms behind my neck. Do you understand the purpose and

extent of the exam. Yes said Ellen.

OK said Shirley I am now going to do the other breasts. Shirley carefully

repeated the exam of her other breast just as completely as the first one.

She said Ellen I now need you to stand up. Ellen stood up, and Shirley

showed her the leg extension of the table. Shirley got on the table and laid

on her back and ask Ellen to pull out the leg extension. When it was out to

support Shirley’s legs, Shirley said I am now going to repeat the breast

exam while laying down. This is just the way a doctor or I do them. So

Shirley did the exam of each breast ending with squeezing the nipple while

Ellen watched. She again ask Ellen if she had any questions and Ellen

said no. Shirley said there is one more position in which many doctors are

doing breasts exams now and I will show you it also. Not all doctors are

doing this but the ones who do it says it provides a better way to feel

lumps. Shirley turned over and got on her hands and knees. She then

took her right hand and started feeling her left breasts. She did it in the

same way as the other positions, circular motions starting on the outside

of the breast and circling in towards the center. When she reached the

center she squeezed her nipple. Then she repeated it with the other

breast and nipple. She ask Ellen if she had any questions about this

position and Ellen said no. She asked Ellen to push in the leg extension

and sat up and the end of the exam table with Ellen standing in front of

her.

Shirley said Ellen do you want to feel my breasts and do a breast exam on

me. Can I asked Ellen. Of course you can said Shirley. Start with my right

breast and I will put my arms behind my head in the proper place. Ellen

then very tentatively touched Shirleys breasts on the outside and started

moving around. Shirley said stop for a minute. Use the pads of your

finger tips and push in lightly. You want to feel the skin but also you want

to be pushing hard enough to feel any lumps. So start over and do my

entire right breast and nipple. Ellen started over and this time she had the

right technique. It took her about 3 minutes to do the breast and to pinch

her nipple at the end. I didn’t see any discharge Shirley. Shirley said you

did great not do my left breast. Ellen again started on the outside, pushing

with the right amount of force, and continued circling until she was at the

nipple and then she pinched the nipple. There was no discharge from this

one either.

Shirley said Ellen you did a good job. What do you think about doing a

breast exam. I would have no problem doing on on my self when I am

alone. That is the essence of self breasts exams. It is best to do them in

the shower right after your period ends. The shower provides water to

lubricate the breast tissue and lets your fingers slide easier. Will you start

doing a self breast exam every month right after your period ends. Yes

said Ellen. Good said Shirley.

It is also OK when you are home along, to lay on your bed nude, and to

stimulate your breasts, nipples, and your clitoris and see if you will have an

orgasm. If you have one, then wait a couple of minutes and to it again to

have another one. As your are caressing your breasts, nipples, and

clitoris, you will learn what you like and what makes you climax. I will do

that said Ellen. But that is what my Dad told me not to do. Shirley said

year said Shirley. And every month I do a self exam of my breasts. Yes

said Shirley. Shirley do you do these exams. Of course said Shirley. Dr.

Novak is my doctor and he does my complete physical exam every year.

And I do self exams every month. A couple of times I felt a lump while

doing my self exam and went to Dr. Novak to have him check my exam.

Both times it decided it was no large enough to do any thing about it yet,

but he checks these spots carefully at each follow up exam.

Shirley you told me that Sam will like my breasts more when he can caress
