For the first time in a couple of weeks Shirley woke up not thinking about Anna and Jim. Today she was thinking about Dan. How surprised she was when he invited her to dinner. Jane I am now ready to do the pelvic exam. I need you to put your legs in the knee crutches and scoot your bottom down to the end of the table. Shirley helped her get her legs in the knee crutches and then told her to scoot down a little more. Shirley put on gloves and lubricated her fingers and Jane’s vulva. It is a plastic Peterson speculum. It opens the vagina vertically so that I can examine it, see your cervix, and do a pap smear. Jane said it is so big. It is the smallest one they make and is made specifically for girls like you who are still virgins. I will lubricate it well and insert it gently. After it is inserted I will rotate it 90 degrees so the handle is down and then expand it. You will hear a clicking sound when I expand it and you will feel your vagina expanding. Are you ready and Jane said yes. So Shirley carefully and gently slipped the speculum in, rotated it, and started expanding it. To her surprise Jane didn’t say anything. I need to do your pap smear now. Are you ready, yes said Jane.

Shirley showed Jane the wooden tool used to collect the first sample. She inserted it into her vagina, put it on the cervix and rotated it three times. She then took it out and went to the counter and rubbed it on the slide and

sprayed the fixation spray on it. She then took the small brush and showed it to Jane. She explained that she will place it in the opening of her cervix and rotate it to obtain samples. It should not hurt but you may feel it. So she inserted it into Jane’s vagina and the opening of her cervix and rotated it three times. She took it out and rubbed it on another slide and sprayed the fixation spray on it. She took the speculum out of Jane. She then showed Jane a metal speculum. She explained that it is a Collins speculum and it opens the vagina horizontally. It gives a better view of the top and bottom of the vagina and the urethra. After lubing it She inserted it and opened it all the way. She got the mirror and had Jane look at her vagina and cervix again. She also looked at her urethra. She again asked her if she had any questions and she said no. She took the Collins speculum out and put it in the sink. While the Collins speculum was in she had Jane feel the top of her vagina. She ask her if she felt part that was different and she said yes. She said part way in it feels firmer and it feels good when I touch it. As Shirley was reviewing the chart Jane said I guess there is something else I should tell you. What is it said Shirley. Jane said I forgot all about birth control. I mentioned it to the nurse this morning and she said Dr. Novak could give me an injection and it would protect me for 90 days.

Can you give me the injection. Yes said Shirley, but it is really two injections, one in each cheek. It is 100% effective but I must tell you it stings quite a bit. Jane said at this point I don’t have any choice so give me the shots. So Shirley told her to turn over and lay on her stomach again. Shirley prepared the injections and returned to Jane’s left side.

She cleaned her skin and poked a needle in her left cheek. Jane cried out. Shirley said I am going to start injecting the medicine now and it will take two minutes. It is going to sting a lot. As Shirley started pushing the plunger, Jane started crying. When Shirley finished she pulled the needle out and put a bandaid on Janes’s left cheek. Jane was still crying when Shirley went around to her right and stuck the needle in. As she started injecting the medicine Jane yelled out this really hurts.
