So I just found out that I can look at my charts from my doctor online. I was able to read my last physical and what the doctor and nurse wrote down. Its really cool but I really don't want to share exactly what it said but here is a summary. list came from a form I found, actual answers are from my chart. anyone else read their charts?

Abdomen:
Observation
Skin: nice even color, few freckles. no suspicious moles or discoloration. umbilicus inverted but normal.
Symmetry/contour: completely symmetrical, smooth contour. slight definition
Visible peristalsis: slight visible aortic pulsation above umbilicus
Auscultation
Upper left: present/normal
Upper right: present/normal
Lower right: present, slight hyperactive
Lower left: present/normal
Vascular: no bruits
Aorta: strong pulse
Renal: no bruits
Iliac: no bruits
Femoral: no bruits
Bowel sounds: present, active. increased in lower right abdomen
Percussion
Liver: normal, palpable lower edge
Spleen: normal, palpable due to patients thinness
Murphy’s punch: normal
Percussion note: tympanic through out abdomen, some increased tympani left lower belly might indicate abnormality
Abdominal tone: normal with slight natural resistance
Skin hypersentitivity: negative
Light palpation: no tender areas found, no organomegaly detected
Deep palpation: slight organomegaly of cecum. possible constipation/meteorism. no other organomegaly detected.
Inguinal lymphnodes: normal
Hernia evaluation
Umbilical: evaluated due to slight inversion of umbilicus. negative palpation and cough test
Inguinal direct: negative
Inguinal indirect: negative
Femoral: negative
Acute abdomen special tests
Rigid abdomen: negative
Rebound tenderness: tenderness in lower right side in area of caecum. also tenderness around umbilicus. possible small bowel inflammation
Valsalva: Negative for hernia in abdomen
Murphy’s sign (GB): negative
Murphy’s punch (kidney): negative
Carnett’s test: negative
