Examination forms

Hello everyone,

Do you know where I can find (genital) examination forms for medical exams of males?

Commonly, those forms are stupidly simplistic. There may be a line drawing of genitals to mark any findings upon but more commonly they're just a line on the physical exam form - GENITALS with a tick box for within normal limits.
