I had some good ideas that would keep things exciting:
As part of a play scenario, you could pretend that your female's doctor has retired, and that she needs to go to a new practice. There are 4 doctors at the new practice, and they want her to see all the doctors at least once so that they are all familiar with her condition. You could make up a different personality for each doctor: one could be a stern old school doctor, one could be charismatic and charming, one could be nerdy and socially awkward, and one could be young and nervous. Each doctor would have a different exam style: One might give the patient a paper gown, one a cloth gown, one a cloth gown w/drape, one might take the gown after the breast exam and not offer it back. One might do a lengthy external, one might do a full seated/lying breast exam, they might use different types of speculums, and they may or may not do a R/V, etc.
Additionally, the female patient could make up 4 alternate versions of herself, the same Jane Doe, just slightly different:: One who is naïve and submissive perhaps a college grad student, one professional and classy businesswoman, one fun and easygoing soccer mom who is perhaps a tease, and one who is a sexy but vulnerable woman who just went through a divorce. One could have her hair pulled back w/ full pubic hair, one could have a wavy/curly hair w/ trimmed pubic hair, one could have messy hair with a landing strip, and one could have straight hair a shaved pubic area, or you could mix and match to portray whatever "version" of your patient. The could all dress and accessorize differently: One might wear a dress, leggings, and high boots, one might wear a suit and heels, one might wear yoga pants and a sweatshirt, and one might wear sweatpants. The jewelry can also be different: everything from eye catching necklaces, dangling earrings, clip on nipple/belly button/clit rings, toe rings, etc. The idea for the patient is to appear to be an alternate version of yourself for the exam., i.e.: What if you, the patient, had made different choices in life, and you were the same but also a different person, i.e. what if you became a business woman, went to law school, had 4 kids, etc?Your partner, the doctor, then won't feel like he's cheating by examining another woman.
Of the 4 doctors, one could have the potential to "go to far" under the right circumstances, and of the 4 patients, one could secretly want the doctor to do something inappropriate. One of the doctors could also be open to an affair outside the office, and one of the patients would be receptive, but if you were the Dr you wouldn't know what patient is the one, and vice versa.
You would then have 16 possible scenarios for the 4 doctors and 4 patients, and many aspects of each scenario would be a surprise, as you wouldn't find out until the actual exam. You could schedule an exam once a month and play out these scenarios indefinitely. I have a lot more ideas for play exams (including detailed scripts) that I'll share if anyone is interested...
