To give a brief history, I had surgery to untether my spinal cord a couple years ago. Since then at times I have struggled with pain in the penis, testicles, perineum, and prostate at times, a long with occasional urinary retention issues.
Recently another incident of pain and urinary retention arose. After several hours of a really full bladder, I went to the hospital. I was catheterized with an 18fr foley, given pain meds, and examined. Within 30 minutes, about 2000 mL of fluid was drained from my bladder. I received an ultrasound of my scrotum and testicles to rule out any torsion or other issues. I was then discharged with the catheter in and referred to a urologist.
Bladder spasms occurred and were quite intense, so my roommate took me back to the ER where I was placed on spasm medication. Finally after a week it was time for my visit to the urologist.
I went to the urologist still catheterized, with my leg beg. I was asked by the nurse to strip from the waist down. The nurse undid the leg bag to take a urine sample directly from the catheter, she then connected me to a bed bag, so that any urinary output could be measured if necessary, and had me lay down on the exam table to wait for the doctor. She attempted to cover my lower half with a blanket.
The doctor then came in. He asked me my previous medical history, and proceeded to ask me questions about my normal bladder activity, sexual activity, and other embarrassing questions. Then he carefully examined my testicles, scrotum, and penis. The tugging of the catheter strung a bit in my already irritated urethra. Putting on a new pair of gloves, and grabbing lube, he had me flip over for a prostate exam, which was not very pleasant with a catheter passing by the prostate. He then told me that his assistant would come in to remove my catheter, which would stay out, providing I could void the fluid they would fill my bladder with.
The assistant came in with the doctor still in the room, and she disconnected the collection bag, untaped the catheter from my leg, and deflated the balloon. She removed the catheter clamp and pumped 500 mL of a sterile solution into my bladder. The cold liquid going in my bladder was an extremely strange sensation. As my bladder shrank while catheterized, I began to feel extremely fully. Soon enough she then removed the catheter, which strung. I did feel slightly embarrassed with the doctor watching the entire time. They gave me a plastic urinal, and instructed me to attempt to urinate. I was quite embarrassed when they gave no indication that they would leave the room. But with my bladder feeling full, I emptied it in front of them, with 500 mL coming out into the urinal. The doctor was satisfied and said that the catheter could stay out.
His assistant then left the room. He inspected my penis again and applied some type of cream to the irritated tip of the urethra. He also gave me a pill of some sort to prevent any type of infection. He then expressed concern that prostate issues may be playing into my problems, so a more thorough exam would be necessary.
I bent over and he lubed up and again inserted his finger in my rectum. As he touched my prostate, I started to get erect. He informed me that he would like a semen specimen as this could rule out any abnormalities in addition to the urine sample. The doctor stated that since my penis was irritated from the catheter, stimulation of the prostate would provide the best sample. I did not like to hear this, but didn't argue, as I just wanted to go home at this point. He held a specimen cup below my erect penis and began to manipulate my prostate. I felt very embarrassed and violated. I ejaculated quickly and he removed his finger. He wiped up my anus with a towel and allowed me to clean myself up.
He then told me to get dressed and that the receptionist would schedule a urodynamics test for me in a few weeks and then a follow up appointment with him. Very interesting appointment. I am a little nervous about the test and follow-up, but I won't lie, I am also slightly aroused by the humiliating nature of the procedures.
