I would say my 1st (and only) physical exam with a nurse practitioner where I used to work.
As part of my employer's ongoing cost cutting strategies, they went from a plant physician onsite to a nurse practitioner. We had been given complimentary annual physicals, which was great because you didn't have to take time away from work to schedule an annual physical. And for someone in as good health as I was during my 20's, 30's, and 40's anyway, it would have been a waste. I probably wouldn't have even HAD physicals, Since I didn't even have or use a regular physician during that time, and was in perfect health for the most part.
So before the year where they took one more cost cutting step, and just had me meet with the NP in her office to go over my blood work and urinalysis, & be lectured to have my own physician & go for physicals on my own, there was the one year where she actually physically examined me. In previous years, I'd be told to leave my briefs on. On this one occasion where she took me to the exam room from the waiting room, she said something like "Everything off except your shorts. If you want your genitals examined, then everything off."
This was the 1st offer I had had in the 20+ years I worked there to have my genitals examined. For the previous several years, a gown was always put out on the exam table for me, but i never wore it. Because when i had started there when there was a male plant physician, I was in briefs only. So i never bothered to wear a gown from the time they started putting them out and he was still the plant physician.
So now when she said "..if you want your genitals examined...then everything off", I still wasn't going to wear a gown. But I felt funny, so I used it as a drape over my genitals as I sat there waiting. When she had me lie back on the table, a few times it wanted to slide off, and I had to catch it.
But it was the only time I can recall that I stood before a medical professional totally nude, no briefs at my knees or anything. She held and examined my penis, the only medical professional that has ever examined it, or touched it really. And she examined my scrotum and testicles as well, before giving me a rectal exam.
Aside from docs who have checked my testicles during a physical, the runner up for intimate exams has to be my semi annual skin exams, where my dermatologist one time touched my buttock, pointing out where it was dry and should have moisturizer applied to it. I would consider being alone in a room with 2 women (her assistant) in just a jockstrap, no gown, to be somewhat intimate.
