I always like doing the genital and rectal last. I start off with the hernia check and most guys are expecting that, but not expecting to have their penis examined or have their scrotum thoroughly examined. And then comes the prostate and rectal exams 😃
For me genital examination is usually done last but I remember a physical at my pediatrician that was literally from head to toe. He make me undress completely and lie on the examination table, started to check mouth and throat, auscultation of heart and lungs, abdominal palpation then I get a testicular palpation, penis was retracted and examianted accurately. After genital examination the doctor continues on the knees, legs and feet checking and measuring principal lengths.
For me, at the pediatrician, the genital exam was generally done near the end of the exam. There was one time, however, when I was about 12, when she decided to change the order somehow: after checking height and weight, she made me lie on the examination table and loosen my belt, she started checking my abdomen, and then, almost without warning, undid button and zipper of my trousers, pulled them and underwear down a bit, so that my groin was exposed, and started to check my genitals. Since it was my first time with her since the onset of puberty, it took a bit longer than normal: she started noting hair presence and distribution, then went on to the testicles, and a thorough examination of my penis, including retraction of my foreskin (turns out, it was a bit tight).She then moved on doing what was usually done first: checking my heart rate, which, since she had forgotten to cover my lower regions, was a bit high. I think every other time since was done in a head to toe fashion.

Occasionally, I have had the meatus spread open but usually nothing is done with my penis at all. I am also curious about the "visual inspection" and what it consists of. I like the idea of the examiner inspecting as well as examining. This implies that some judgement is being made as to the adequacy of the penis.
No. Next to last. My doc leaves the genitals to the ALMOST last thing. The last thing is the rectal exam. Genitals are examined by penis first (usually), checking for problems on the tip, then the corona and the shaft. Then he checks the scrotum and testes. Then I have to flip over, bend over and spread. A dab of lube swirled around the rosebud and in it goes. Almost like the army physical, except now the doc doesn't stop with a quick look for obvious haemorrhoids! I have a separate urologist as a specialist who obviously goes straight for the goodies.
For4 those of us who d9ont get rectal exams, they're almost always last. The reason the rectal last is that its always 'clean to dirty,'
Hernia check, testicular exam, palpation of penis shank, foreskin retracted, pee hole inspected either by slightly squeezing the slit with forefinger above and thumb below or both thumb spreading the lips of my urethral meatus (pee hole slit) - I have had it both ways.
My last annual was with a new MD and after all the questions she told me "Well today I'll be doing a rectal exam, unless you prefer I don't" I agreed it should be done and she said "Good I usually do that part last, but we can start there first, if you want". Never had that option before.
