It's funny that you should ask that question. I actually had one doctor that did exactly that. I saw her exactly one time for a complete physical, or rather what she called a complete physical. It was during one of my many times when my employer switched our insurance provider so I was shopping for a new doctor. She was absolutely terrible and didn't do anything even resembling a complete exam. I never went back to her again, since I really didn't have any confidence that she was very thorough.
However, for this exam, I decided to wear a jockstrap as underwear. I remember that the nurse had me remove my shirt for the EKG and that was really it. She handed me a gown and paper drape as she left the room without any further instructions. So, baring any instructions, I decided to remove all of my clothing except for my narrow-band swimmer jock. I hate gowns, so setting that aside, I sat on the exam table with just the paper sheet covering me.
The female doctor came in and did a very cursory exam. When it came time for what I thought would be a genital and hernia exam, she punted and simply asked me if I had any issues in that area. I thought about telling her that it was her job to find out, but I didn't. Maybe I should have. But in any case, the jockstrap was never lowered for a genital exam. But she did say that I was at the age when I needed to have a prostate exam. Yes! I thought to myself. She told me that I needed to stand and bend over the table. She didn't say anything about my jockstrap, so I asked her if she wanted me to pull down my jockstrap. She answered no, that it looked like she could do the prostate exam with it still on.
So there I was, bending over the exam table with nothing on but my jockstrap. She quickly lubed up her gloved finger and slid it into my awaiting hole for a very quick prostate exam. She did have to push part of the jock aside in order to get access to my hole. That's because normally the two rear straps and the bottom of the front pouch are all sewn together at one place, which just happens to be right over your hole.
Because of the prostate exam, it actually turned out to be an interesting experience. But she was a terrible doctor and I never saw her again. I often wondered why she did the prostate exam but neglected to do a genital or hernia exam. Maybe it was the jockstrap that threw her off her normal routine. Maybe she read too much into the jockstrap and thought that I was too embarrassed to have the front pulled down, but I was OK with the prostate (since my ass was already fully exposed). I don't know, but I often wondered if the jockstrap had any affect on how that exam actually went.
