I don't know why, but my fantasies always take place in winter time.
My patient is dressed to keep warm. She wears boots, tight-ish jeans, a woolen turtle-neck jumper and underneath it a cotton vest and a plain bra (its colour changes as per my mood -- right now I'm thinking black). She has a coat on.
This is Europe. There are no gowns, and rarely an attending female nurse.
The doctor's office is luxurious, state-of-the art furniture and expensive decor. There is a black leather examination couch. Near it, the EKG and ultrasound equipment. The place is intimidatingly tidy and clean. This is the private practice of one of the top cardiologists in town. The patient feels privileged to have access to this level of medical care, but for some reason these surroundings unnerve her.
She sits down on the chair in front of the doctor's desk. A dialogue ensues. He asks what he can do for her. She describes symptoms (palpitations, usually), he asks more questions. She's nervous and in awe of the doctor - she speaks with her gaze lowered. He is polite but firm. As he gets up from his chair, he tells her to take off her coat and jump on the bed.
She remains fully clothed as he checks her pulse on her wrist. He tells her to roll up her sleeve for blood pressure taking - which he performs with the girl in sitting and standing positions as well as laying down on the couch. He asks her to sit up and remove her top clothes. He turns away as she does so, hesitantly. "Shall I just leave them here," she asks. "Yes, just place them next to you on the couch." Then, a few seconds later, "My bra as well?" and "Yes, please."
She is topless as he auscultates (and boy does he take his time listening to her heart in all possible ways). She is then told to remove her shoes and socks for the ECG. Occasionally, again, depending on my mood, he might tell her to remove her jeans, too - if he thinks they're too tight.
I'm kind of embarrassed to admit it, but I very rarely manage to get to the ultrasound part... If you know what I mean.
