During elementary school I'd get yearly physicals, but they were very cursory and didn't include any genital or rectal exams that I can remember. During middle school age I'd go every year for a required sports physical. It was your typical school sports exam which usually included a hernia exam but not really any other genital exam and never a rectal exam. During high school and college the only time I went to see a doctor was when I was really sick, never for a physical exam. After college I was required to have have two different pre-employment physicals. Both were almost identical to my previous sports physicals for school, including a hernia exam but no genital or rectal exams. Until recently, those were my only physicals, 3 in school for sports and 2 after college for jobs.
About nine years ago or so I hurt my back and during the many doctor appointments discovered I had high b.p. During that experience I settled on a female family practice PCP about the same age as me. She basically required me to come in about once a year to have labs done and renew my prescription meds for my high b.p. Never had to remove any clothes for her, never received what I would call an actual physical. She would look in my throat and ears, then tap on my back in a couple of places, and finally listen to my heart. Literally took less than 2 minutes on the exam table, and maybe 10 minutes total in the room including history and small talk.
Recently I switched to a male family med PCP about 5 years older than me. My first visit with him he gave me a very comprehensive physical including genital, hernia, and my first ever rectal exam. Start to finish, including medical history and questions, it took a little over an hour. Before I left, he told me that I should be getting a physical every year. So if I do decide to stay with him I will probably start having a physical annually.
Also, for the past 6-7 years I do go annually to a dermatologist for a full body skin exam.
