I have have mostly done play exams but if the person was playing the doctor was good at the details and knew what they were talking abut I might consider doing an OR scene. When I play I like it to be as realistic as possible for me its very exciting when they know what they are talking about.

Though I like watching videos about surgery prep but for a regular play exam I would like to have it in a doctor's clinic
I definitely prefer a realistic play exam to be performed in a medical office. I'm not into surgical play or surgical prep play, so the whole surgical setting by itself doesn't make it any better for me.However, I can imagine that as a part of an exam scene, the exam then proceeds to an OR kind of setting for some more demanding or thorough procedure (previously agreed between doctor and patient according to their preference, but not involving unnecessary pain or cutting). Could be a rectoscopy, one or more injections, catheter, or just a very thorough genital exam, even if in reality neither of those would justify the sterility of an OR or the use of surgical gowns, sterile gloves, and surgical mask... I think that the power exchange dynamic can be enhanced by making the environment more "sterile" (especially in the sense of cold). But it wouldn't be appropriate for a regular play exam, at least for my taste.
For me, it just needs to be a neutral space, i.e., a hotel room. A person's home is too, well, personal. A hotel room is designed to have general appeal to anyone who stays there--neutral walls, none-too-flashy decor. They're more like an office with a bed and shower than a traditional domestic space. In the times I've tried doing a role play exam either in my home or the play doc's, it's almost impossible for me to get into the right headspace because the room so tailored to that individual.And like others have pointed out, waiting for someone who has a legit medical practice and is willing to play in that space would be next to impossible to find. (Or at even a role play doc who has a dedicated room in his home for play exams.)
To receive a very thorough exam in the exam room and then being restrained to an operating table (and may be mouth-gagged to make me unable to protest) in the operating room for some unnesessary pain experiments would be perfect for me.
I like the doctors office but would be open to an operating room or procedure room . It would depend on what needed to be done. Both as a play doc or a patient.
Definitely not an O R but might go for proceedure room.too many things to do to get into a real sterile area.
If I were to roleplay, it would be in a doctor's office or similar. The only roleplaying I've ever seriously contemplated revolves around fit testing an immunosuppressed or anaphylactic female patient for a mask to wear in public.
A couple of years ago i would have said doctors office. But recently I had a surgery where i was mostly exposed, partialy restrained and it was incredible. Even at the point when i was being put under and started to feel like i didnt have control of my breathing wih the mask etc... it has really awakened my interests.
I would Love to be in the operating room as a Patient and be strapped down on the table waiting for the mask to be lowered over my face and ready for the doctor to do what he or she is going to do to me.
Long back in this topic I have said that I prefer doctor's office. That was one of my first few posts and I was new to play exams. But now after six months on Zity I have read others experiences as well and my surgery fantasy has developed. Now I have equal preference for doctor's office and OR. Infact it would be even better if both can be combined.
I would love for the experience to start as an exam, but then move to a surgical procedure. My preferred room would be one that combined elements of both an office and an operating room.

@camus that depends on how good the play doctor can link your exam to a surgical scenario. Like for me tummy exams are favourite so it would be great if the play doctor can link it to diagnostic laproscopy of my tummy.That's the approach I can think of. It may depend on where and how the play exam progresses.

I like be into the operating room and the nervous scary feeling of the clinical atmosphare. The sounds there and being watched and examine from many masked and scrubed peoples.

Definitely an exam room/doctors office.Personally, I don't find operating rooms and surgery to be the least bit erotic.
