Orgasms in age play

I'm curious how other age players incorporate genital stimulation/orgasm into their play, or if they do at all. I'm definitely a sexual age player, but also like to feel embarrassment and naughtiness around "private parts" when I'm in that headspace (of course, not generally), which means I like to experience these things much differently than I do when I'm not “playing”. A couple of examples are being masturbated over a lap as part of a spanking, perhaps while being scolded, or by a nurse or doctor as part of an intimate exam.
