I had my second knee replacement last fall. When I had the first one done, my pre-op physical was done by our internist, so it followed the same pattern that I've written about in other posts.
I expected the same to happen when it came time to have my second operation. This one turned out a little differently, though. Dev and I drove to Boston to meet with the surgeon to finalize the plan and select the date for surgery, which was to be done on an outpatient basis in New Hampshire at a clinic where my surgeon is part owner. Because of COVID, we were able to set the surgery date for about three weeks out, as many people seem to have opted out of surgery for the time being.
After we got done setting everything up with the surgical scheduler, we were directed back to the surgeon's office. This was different from the first time, where the scheduler provided us with all of the forms to take back home to our PCP, etc. and sent us on our way. The doc closed the door and said that he wanted to do my pre-op physical, since we were within the 30 day time limit. He asked if I wanted a chaperone or if Dev would suffice. I told him that Dev would do fine, and he left the room, saying he'd send one of the MAs in. There's an exam table right in his office. The MA came in and drew my blood and had me go into an adjacent rest room to provide a urine sample. When I came back into the office/exam room, she'd laid a fabric sheet/drape on the table. She told me to take everything off but my bra and panties, hop up on the table and cover myself with the drape and she'd send the doc back in.
So I stripped off, even my socks, which I hate to do, leaving my bra and panties on as directed. I plopped down on the table and sort of arranged the drape around my lower half. I figured that it wouldn't be an extensive exam, since I still had my bra and panties on. The doc came back in and said something to the effect of “let's get this done so we can get you guys back on the road to Maine.” He did the usual eyes, ears, nose and throat stuff, taking quite a long time looking in my mouth and throat. Dev later told me that this was to assess my airway and tongue as part of the pre-anesthesia workup. The CRNA later did the same thing on the morning of surgery. Then he felt my thyroid and the lymph nodes along my jaw and neck.
Next, he asked me to remove my bra so he could listen to my heart and lungs. I did so and tossed it over to Dev, who dutifully folded it and put it with my clothes. The doc percussed here and there along my spine from my neck to my bottom and then percussed and auscultated my lungs from the back. He moved to the front of me and listened to my lungs, then my heart. He had me lay back on the table and listened to my heart again with me supine and then took the stethoscope out of his ears and asked me to raise my hands above my head. He did a very quick breast exam, with the key word being quick. Then he bunched the drape up over my breasts and auscultated my abdomen. He percussed my abdomen and then palpated all four quadrants. Then he sort of peeled my panties back a little and felt my femoral pulses and lymph nodes. Next, he checked the pulses behind each knee and on top of both feet. I figured we were done at this point. But he asked me to remove my panties. As I did, he put gloves on, so I figured I was in for something more thorough. He had me get in frog-leg position, which wasn't easy with a bum knee, so Dev came over and supported my bad knee. The doc did a quick external exam of my vulva, then lubed two fingers and did an also brief pelvic exam. Then he told me to roll over onto my side and pull my top knee up. I knew what was coming. He did at least change gloves and use a lot of lube before he did a rectal exam. He took off the gloves and handed me a box of cheap tissues, telling me that I could get dressed and he'd be back in shortly as he left the room.
While I got dressed, I asked Dev why the doc didn't focus more on the orthopedic stuff in the exam, and Dev told me that he already knew pretty much all he needed to know from seeing him several times in the past year.
The doc came back in and said that everything looked good and he'd call me if the labs came back abnormal and to check the portal for the results.
So we headed back to Maine, had lunch at Bob's Clam Hut in Kittery, and went home, where, as usual after an appointment, Dev had his way with me. Or maybe I had my way with him. 😉
