I can be both. Perfer erotic but if my patience want realistic I can do that as well.

I like to give and get a bit of a blend... pretty realistic but with some emphasis on more erotic areas...and play it up or down based on the play partner. But it's important that it's grounded in some reality and not just a thin veneer over sexual play.

I agree with tempnyc2001 I like there to be a lot realism but with some "additional" procedures in the mix.

I believe in being as real as possible in the beginning but adding erotic twists to the exam. Definitely more fun to start off clinical and then lead into being more erotic as the exam goes on. Depends on what the patient prefers.

For me, the playing doctor is an erotic entertainment, so I prefer an erotic access to the affair. As a retired physician, I have done thousands of real exams. Thoough I have done only a few play exams, they all must come from the real framework that is very much part of me. I would not have thought of my play exams as erotic in the sense that I think of as eroticism. But there is no doubt that in the playtime world, I would certainly allow myself some liberties I would NEVER have taken in a real exam setting.

For example, I might linger over a rectal temperature or give a "needed" enema, neither of which I ever did in practice. If that's erotic, then I guess I would now be predisposed to an erotic exam.

Ptfordoc I think the majority of people would like any play exam to take a traditional realistic exam and then expand on it, its part and parcel of the medfet fantasy/kink. Obviously afterwards leading on to a full body exam, ending with my lower clothes being asked to be removed. Then a very detailed internal exam, including smear test, std tests, whilst asking more probing questions.

The level of exam would progress into my labido performance, under strict scientific conditions the Dr would need to assess my levels of stimulation, examining me internally at intervals with his fingers, and a speculum to continue to monitor the strength of the building climax.

Finishing with a clitoral examination and vigorous stimulation until an orgasm is witnessed, and recorded.
