Sarah goes over Arielle’s family and medical histories, updating her chart from her last visit almost three and a half years ago. Now updating Arielle’s gynecological history, Sarah asks questions about her periods, whether she has any issues such as irregular periods, bleeding or spotting between periods (no), any issues with PMS symptoms (usually no, sometimes mild cramping), and if she has any issues with the hormonal contraceptive she is using for birth control. Arielle says “I would like to explore a different non hormonal birth control method, I’m not really having any issues with the pill, but I don’t want to use hormonal birth control for years and years.” Sarah says “I understand your concerns about years, even decades of use of the pill. You are on the lowest dose possible while still maintaining effectiveness against pregnancy. There are always going to be risks using the pill, the risks are small, but still are risks nonetheless. I usually recommend the non hormonal IUD as an alternative to hormonal birth control methods. It is almost as nearly as effective for pregnancy prevention as a vasectomy or tubal ligation. And if you ever want to have children, as soon as it is removed, you can get pregnant almost right away. Since I will be giving you a pelvic exam today, I will be able to tell you if you are a good candidate for it. If so, we can schedule an appointment for having it inserted. I have inserted many IUD’s, and almost all of my patients that have one love it. No pills to have to remember to take, no paying for refills every month, no hormonal side effects, no having to plan ahead for sex like you would if using a diaphragm, etcetera. Very few of my patients have had to have it removed because of intolerable side effects like bad cramping that doesn’t resolve within a few days after insertion, for example.” Arielle asks “How much does it hurt to put in? I haven’t even had a speculum in me before!” Sarah replies “There will be some discomfort, some women might even describe it as brief pain, especially as it is inserted into your uterus through your cervix. I would have to use a couple sounds to dilate your cervix for the insertion, which is usually the worst part, but it is over quickly. This is the reason why we typically schedule the insertion for when you are menstruating, when your cervix is already slightly dilated, and any bleeding from the insertion just comes out with your usual flow. I can give you a pamphlet that describes the procedure in detail, and what we do to help minimize any discomfort or pain.” Arielle says “Okay, I can read up on it and decide later.” Sarah says “If you decide to go ahead with it, just call and book an appointment with me for when you are on your period. I will go over everything with you before we begin, and we will take it slow to be sure you are as comfortable as possible. If it helps you, you can bring Adam with you for moral support and a hand to squeeze as hard as you can.” Arielle and Adam laugh a little and Adam says “She does have a strong grip, I hope she doesn’t break my hand!” Sarah and Arielle laugh and Sarah says “We can put a cast on your hand if she does. Now Arielle, any questions before we begin?” Arielle replies “Umm, no, not that I can think of right now.” Sarah says “Well, don’t hesitate to ask if you do. Now I will have you take this cup and go down the hall to the bathroom and provide a urine sample. When you are finished, leave it in the cubby in the wall, just pull open the little stainless steel door to access it.” Arielle stands up, takes the cup that has a screw on cap, and already has a label with her name and patient number affixed to it, opens the door and heads down the hallway. While Arielle is gone, Sarah removes the used table paper and stuffs it in a large trash can in the far corner of the room by the scale. She then uses a sanitizer to thoroughly wipe down the exam table, the Mayo stand (after throwing away the paper towel that had the lube on it), and other high touch surfaces near the exam table, finishing with the countertop. She then pulls down fresh table paper from the roll mounted to the head end of the exam table, tucking it under the thin strap at the foot of the table that holds the paper in place.

Arielle finishes filling the cup, stops her urine flow to remove the cup from underneath her, and sets it on the small shelf next to the toilet. She finishes emptying her bladder, then wipes herself. She sees a box of tissues on the small table next to the sink. She takes a few and lightly dampens them with water, and wipes herself thoroughly. Arielle slips off the exam table, making more paper crinkling noises, and sits in the chair next to Adam. Sarah removes a plastic speculum from the top drawer at the foot of the exam table, still in its wrapper. She unwraps it, then takes a small battery from a charging stand on the counter. It has a small LED light on the top, Sarah partially inserts the battery into the handle of the speculum, the light not lit yet, and places the speculum on the stand next to the cup. The speculum looks big to Arielle, but she will soon find out that it is “only” a medium Pedersen and not a large Graves! Sarah removes a tube of lube from the same drawer and closes it, and steps aside, telling Arielle she can have a seat now.
