Shirley was ready about 8:45. Once again she was wearing a sun dress. Sun dresses had become her standard clothing. She really liked the feel of her nipples on the fabric of the dress and not wearing panties. Right at 9:00 the doorbell rang. She went and opened the door and there stood Poly and Ron. Its time for the breast exam said Shirley. Poly said why are you going to do a breast exam as breast cancer will not cause a fever. Shirley said when I am done I will call Dr. Novak and he will asked me if I did a

complete exam. If I say no, he will tell me to do a complete exam and call him back. So put your hands behind your head. Shirley then started felling Poly’s right breast with the tips of her fingers. When she finished feeling her entire breast she pinched the nipple. Shirley then did the left breast the same way. Shirley told Poly to lay down on her back and repeated the breast exam. Next Shirley told Poly to get on her hands and knees and she would examine her breasts in that position. Poly said she had never had her breasts examined in that position and Shirley said it is a newer position but will allow the examiner to feel lumps that can’t be felt any other way. So Shirley again used her fingertips and felt all around both breasts and pinched both nipples. Shirley told Poly her breasts were normal and to turn over and lay on her back. Shirley then palpated her abdomen and ask Poly if it hurt and Poly said no. Shirley did not feel any masses so she got the stethoscope again and listened to Poly’s bowel sounds. Again Shirley said the bowel sounds are normal. Poly I need you to scoot down so you bottom is at the end of the table and put your legs in the knee crutches. Poly did as instructed and Shirley got out a Peterson speculum and lubricated it. She also lubricated Poly’s vulva. First Shirley spread Poly’s vaginal lips and examined her vulva. Shirley said your urethra is red which may indicate a urinary track infection. Shirley inserted the speculum and expanded it the full amount. Shirley looked in at Poly’s cervix and it looked normal. All this time Ron with watching everything Shirely did. Shirley ask Ron if he wanted to see Poly’s cervix and he said yes so he took a good look at it. Shirley took the Peterson speculum out and said because your urethra is red I am also going to use a Collins speculum so I can get a better look at it. She got a Collins out of the drawer, lubricated it, inserted it and expanded it to the fullest. Shirley then took a good look at Poly’s urethra, the walls of her vagina, and her cervix. Shirley said the only abnormal thing is your urethra is very red. She took the Collins speculum out and put on a new set of gloves. Shirley told Poly she was going to do a bimanual exam. Shirley put two fingers in her vagina and then started feeling for her ovaries and uterus. Shirley said they all feel normal. One last thing said Shirley, I am going to do a rectal exam. Shirley put on a new glove, lubricated it, and inserted in Poly’s rectum. She felt all around and noted nothing unusual. She took her finger out, took the glove off, and threw it away. She then told Poly to get up that she needed a urine specimen. Shirley gave Poly a cup and told her to go to the bathroom and fill the cup. Soon Poly came out and handed Shirley the cup. Shirley had gotten three different test strips out of the cabinet and one by one dipped them in the urine. The strip for

bacteria showed positive. Shirley told Poly to sit back on the exam table while I call Dr. Novak.

Shirley called Dr. Novak on the speaker phone and told him of Poly’s symptoms and the results of the physical exam. Dr. Novak said I think she has a urinary track infection and he ask Shirley if she had any rocephine. Shirley said yes she had four doses. Dr. Novak told Shirley to give Poly two injections of one dose each, one in each cheek. Dr. Novak also told Shirley that he needed a sterile urine specimen and asked Shirley if she had any catheters. Shirley said yes. Dr. Novak told her to get a sterile urine specimen and bring it to my office this afternoon. He repeated to give Poly the two rocephine injections and to have Poly come back on Monday morning for another complete exam and to call him with the results. Shirley hung up the phone and said Poly did you understand what he said. Poly said yes, but I don’t want any more shots. Shirley said that rocephine is the best antibiotic for a urinary track infection and it is only available for injection. Shirley told Poly to lay down on her back and put her legs in the knee crutches again. Shirley went and got the catheter tray, opened it, and cleaned Poly’s vulva and urethra. Shirley then put on the sterile gloves and picked up the catheter. She instructed Ron to get two cups from the drawer and told them when the catheter goes in urine will start to flow. Catch the first small amount in one cup, and when I tell you switch to the other cup, and when it fills, go back to the other cup. So Shirley inserted the catheter and Ron did as instructed. Shirley pulled the catheter out and threw it away. She took the cup with sterile urine from Ron and sealed it. She told Ron to go dump the other cup in the toilet and throw it away.

Shirley then told Poly to take her legs out of the knee crutches and lay on her stomach. Shirley then got the rocephine out and mixed it with sterile water. She explained that she had to put each vial on a vibrating roller to get it mixed completely. After five minutes it was ready and Shirley drew the rocephine up into two syringes. She took a wipe and cleaned a spot on each of Poly’s cheeks. She made sure to be at least two inches from the spot where she gave Poly the birth control injections. She ask Poly if she was ready and she said yes. Shirley told Ron to hold her hand and rub her back. Shirley pushed the needle in Poly’s left check and started injecting the rocephine. Poly started crying again and Shirley said I know it hurts. When Shirley finished the left cheek she moved to the right side and repeated the process. Poly was crying the whole time. Shirley put a

bandaid on her and told her when she was ready she could get up. Poly laid there for about five minutes before she got up. Shirley explained that with Poly having a UTI that they could not do the enemas and anal intercourse today and Poly said she understood. Shirley said you heard Dr. Novak, you need to be back her Monday morning so I can examine you again and report to Dr. Novak. Ron said I have to work Monday but Poly is off. Shirley said that is fine as only Poly has to come. Shirley told them they could get dressed. They walked hand in hand to the table with their clothes on it. Ron hugged and kissed Poly and said I am sorry you are sick and had to get four shots. Ron dressed Poly and then dressed himself. When they were dressed Shirley told them that when Poly was well they could come back and do the rest of the session. She also explained that after the second session that she didn’t charge for additional sessions. Ron ask what is the topic of the additional sessions. With these three different kinds of bacteria the bacteria will still be growing and Poly will be much worse on Monday morning. Where does Poly live. On the outskirts of our city said Shirley. Can you call her and see if she can come in later his afternoon. Dan looked at his watch and it was 2:30. Please call her and asked her if she can be here at 5:00. Shirley dialed Poly’s number and Poly answered the phone. Poly this is Shirley. I am at Dr. Novak’s office and he just looked at your urine specimen under the microscope. There are actually three totally different kinds of bacteria in your urine. With the two injections that I gave you, by Monday you are going to be a lot sicker because the bacteria will grow faster than the rocephine can kill them. It is imperative that you get some more antibiotics soon. Can you come to Dr. Novak’s office at 5:00 this afternoon? Ron and I had other plans Shirley. Dr. Novak spoke up and said Poly if you don’t get additional treatment this afternoon, You will be in the hospital by Monday. So it is very important to get the treatment this afternoon. Ron was on the phone also and he said we can cancel our plans. Getting Poly well is the most important thing. We will be there at 5:00.

Well Shirley that worked out well. We have 2 1/2 hours before they get here. The office is locked and no one else will be coming to the office today. Well said Shirley I have been wanting to ask you to give me B12 and B Complex injections. I don’t feel sick or anything but I could use more energy. Will you give me those injections? Shirley said those were not too bad.

Dan said unlike the birth control medicine you didn’t cry. I am glad, I don’t ever want to make you cry. Shirley I have a plan for this afternoon but I need you to turn over and put your legs in the knee crutches. Soon Shirley was laying on her back, legs in the knee crutches, and her bottom at the end of the table. What are you going to do asked Shirley. It is a surprise but I know you will like it. Dan went to a drawer and got a blindfold. He went over to Shirley and put it over her eyes. You can hear but not see until I tell you is is OK. Fine said Shirley. We will see said Dan. When the water was warm Dan filled the 4 quart enema bag and then he put some peppermint soap in it. He opened the clamp over the sink and let the air out of the tube.
