As this middle-aged male enters the room, dressing in a plaid button-up shirt, khakis, brown dress shoes, and a lab coat buttoned halfway up his body. He closes the door and greets Taylor, seated on the exam table. “Hello! You must be Taylor. Nice to meet you.” As he extends his hand for a handshake. Taylor shakes the man's hand. As Dr. Smith sits on the stool, he proclaims, “I understand that you are here to become a new patient of mine. Thank you for choosing me as your doctor. I’m just going to review your chart here, and then we’ll begin your exam. “

After a few minutes of browsing through Taylor’s chart and asking him some clarifying questions, Dr. Smith turns to face Taylor and queries, “Do you have any questions for me?” Taylor states, “No, not that I can think of.” Dr. Smith replies, “Good. If you think of anything, feel free to ask.” Dr. Smith walks over to the sink and washes his hand. He then walks over to the exam table, grabs the otoscope off the wall, and places it in Taylor’s ear. After inspecting his right ear, he switches to his left ear, inspects that side, and removes the plastic tip. He walks over to the counter, removes a tongue blade from the jar, returns to the exam table, and instructs Taylor, “Open up and say ahhh…” Taylor opens his mouth and does as instructed.

Dr. Smith discards the tongue blade in the trash, replaces the otoscope on the wall mount, and grabs the ophthalmoscope. He instructs Taylor, “Looks straight ahead.” He shines the light in Taylor’s right eye, then the left.

He replaces the scope back into the wall mount, walks over in front of Taylor, informs Taylor that he’s going to palpate his head, and begins, starting with his head, running his fingers through Taylor’s hair, which causes a chill down Taylor’s spine. “Is everything ok?” Dr. Smith asks Taylor. “Yes, sir, just a cold chill,” Taylor replies. The doctor makes his way across the top and back of Taylor’s head with his hands before moving his hands to Taylor's face and palpating his forehead, cheekbones, and the neatly trimmed goatee along his jawline. He then lightly palpates the lymph nodes under Taylor’s jaw, moving down to the front of his neck, around to the back of his neck, then down both collarbones. He returns his hands to both sides of Taylor’s neck, applies slightly more pressure, and palpates Taylor’s throat, requesting that Taylor swallow. Taylor complies.

Dr. Smith then shifts slightly to the right side of the exam table and grabs Taylor’s right arm at his shoulder. He continues his methodical palpation down Taylor’s arm, making sure to feel all around his arm from shoulder to wrist. He grabs Taylor’s wrist and bends his arm at his elbow, pushes his arm backward at his shoulder joint, then moves Taylor’s arm away from his body, revealing the fair amount of hair in his armpit. Dr. Smith replaces Taylor’s arm by his side and focuses his attention on Taylor’s wrist, moving it in all directions before inspecting between each of his fingers.

Dr. Smith then moved around to the opposite side of the exam table, repeating the same exercises on Taylor’s left arm as he did on his right. After completing Taylor’s left arm, he moved around to the front of the table again. He sticks his two fingers out on each side and requests Taylor to “Squeeze as tight as you can.” Taylor complies. Dr. Smith then bends Taylor’s elbows to point his fists toward the ceiling. He again directs Taylor, “Push against me,” as he places both of his hands just below Taylor’s wrists. “Good.” He exclaims as he moves his hands to the opposite side of Taylor’s arms before instructing him again, “Now pull up against me.” Taylor again complies.

Dr. Smith then moves to Taylor’s right side while removing his stethoscope from around his neck and placing it in his ears. He directs Taylor, “Just breathe normally for me. I’m going to listen to your heart.” Dr. Smith moves the cold stethoscope around Taylor’s slightly furry chest. After about a minute and a half of listening to Taylor’s heart, he instructs Taylor, “Take some deep breaths in and out for me while I listen to your lungs.” Taylor again complies with the doctor's request. After six breaths, with the doctor moving his stethoscope around Taylor's chest, he then switches to Taylor’s back and moves the stethoscope across Taylor’s back in synchrony with Taylor’s breathing. After eight more breaths, Dr. Smith removes his stethoscope from his ears and informs Taylor that he can return to his regular breathing.

Dr. Smith then walks over to the counter again and grabs the reflex hammer. He informs Taylor to relax his legs and strikes Taylor’s knees, obtaining the desired result of Taylor’s reflexive response by Taylor kicking his legs in the air. Dr. Smith then places his hands around each of Taylor’s legs just above his ankles and requests the same that he did of Taylor’s arms, “Push out for me.” “Pull in for me.” “Very good.” “Now, if you can lie back for me, I’ll pull this extension out for you to rest your legs on.” He says while reaching between Taylor’s legs and pulling the extension out.
