Upon looking at the details , i noticed that they wanted everything checked and next to each item on the "to examine" list, was a box for comments and for the doctor to answer "yes" or "no", if that part was checked and for the doctor to inital. It was pretty detailed, the form included questions about my general appearance, and about my private parts like if I had lumps in my breasts or what color tone my aerolas were and what my nipples looked like. It also asked about my pelvic area and to comment on any the general appearance of findings during the exam. I nervously brought my form into the clinic and I was quickly called back to an exam from .

I handed the form to the nurse who started filling out my vitals and then she asked me to follow her saying: "time to pee in the cup". She led me to a tiny room just big enough for a toilet and a little sink. She reminded me that this was to be supervised and that she staying right here . I pulled my pants and underware down and she handed me a a moist towellette and said to clean my skin first. I spread my legs and started wiping myself down and then I held the cup up to my pee hole and slowly started going. She was watching the whole time. She told me to stop after I had just filled half the cup and It hurt to stop mid stream. I put the cap on and put the cup on the sink counter. I finished peeing in the toilet and I noticed that she kept watching me, which was odd. I wiped up and pulled my pants up as she continued to watch. And I then washed my hands and she told me to follow her back to the exam room.

As soon as we entered the room, we told me to remove all my clothes including underware and bra and to put a gown on., then she said that she'll be back in with the doctor.

Once she left, I stripped and put the gown in then sat on the exam table waiting. I then heard a knock on the door and they both came in. The doctor introduced himself and said that the nurse will be a chaperone for today's exam.

He approached the table and started checking my eyes ears nose and trust with his light and little scopes. He then used both hands and felt my neck and pushed around down to my collar bone. He then put on his stethoscope and listened to my heart by placing the disk on my breast, he moved it around and started listening to my lungs. My gown was now open in the front and my breasts were half exposed to the nurse.

The doctor very quickly pushed the remaining gown of my shoulders and it pooled to my lap. I didn't expect this much exposure and the nurse was looking at my breasts in full view as I was not sitting in the exam table top less. The doctor ordered me to sit up straight and put my hands on my hips. I complied and he started using both hands to touch me. He had really soft chilled hands and my nipples started getting hard when he put both hands on one breast. He reached up and under my arms too and started feeling me. He did the same to my other breast and I felt vulnerable as my arms were out far enough as though I couldn't really stop this. I was starting to enjoy it. The nurse looked at me then looked down at my fully naked body laying on the table while I felt his hand touch my public area. I looked straight down to see what he was doing and that was when he wanted me to bring my heels to my butt cheeks and then had me relax my knees out to the side. I noticed how I now had my pubic area exposed to the room. The doctor touched my labia briefly and I jumped a little in shock. After touching my labia on both sides and opening me a little while the nurse watched. He asked me to put my feet in the stirrups and to scoot down.

I did so while still completely naked laying on my gown. The doctor sat on his little rolling stool and shined a big light on me. I felt him open my labia with his fingers and he pulled the top of my clit hood up to examine it. He inserted a speculum in me and opened it up, I could feel myself stretching then I felt this sharp pitching deep inside me which was the pap smear brush. Then I felt his fingers inside me while his other hand pressed on my nude lower abdomen. I watched him do this and I glanced at the nurse who had her eyes glued to my vagina. She briefly looked up at me then looked at my breasts. The doctor was staring to finish up and then he slipped his middle finger inside my butt hole. I jumped a little and then he twisted his finger in each direction, then his other hand was in my belly again pushing down. While hoping his anal intrusion was nearly done, he put another finger in my vagina again and then squeezed the 2 fingers together. I nearly had an orgasm . The doctor said he was finished and that he will fill out the form while I get dressed and that I did a good job today in my student physical.

Jess' Breast's are Medium sized breasts, hanging loosely, and her nipples were asymmetrical: one seemed to be inverted. No nipple masses or tenderness upon exam. Skin was clear and her breasts overall look fully developed, pink and healthy with a normal stimulation response.

External genitalia :

Jess' Mons pubis is shaved, but scant dark hair. Her Vulva/Labia Majora have no piercings. Her bartholin glands feel normal with no irritation from Jess upon my digital exam. Her Skene glands have no discharge or mass, and some enlargement on the clitoris was seen, but upon my evaluation, it was non- tender and is a normal response to the exam.

Jess' urethra and vagina was examined and all was normal.

Jess verbally consented to the general and gynecological exam.

Today she was placed in lithotomy position

And her External genitalia was manually inspected and palpated with no lesions noted. A lubricated plastic speculum was inserted into Jess' vagina to visualize her vaginal wall and cervix.

A Brush was used to remove cervical cell sample and culture. Bimanual exam was then performed, uterus was found to be smooth. Natural liberation was noted.

Rectal:

Jess' digital rectal exam was Normal, good spincter tone, no hemmeroids or masses. The Recto- vaginal exam was tolerated with no pain or tenderness .

Jess is a healthy 19 year old female, eligible for university performance and should return for her annual check up.
