Sammy was a strong woman. There was nothing shy or weak about her. She knew how to be confident, she knew how to be strong. You'd have to be if you were in Sammy's profession. Sammy was a clinical instructor, but not just any clinical instructor, one who taught men how to examine woman when they were committed to an institution.

Sammy knew that most men simply lacked empathy to understand what it was like to examine a woman by force. Rather than use medical models Sammy did things herself.

“This job requires empathy and understanding. You're going to see these woman at their worst. They're going to be scared and vulnerable. You're going to be forcing woman to remove their clothes, expose their breasts, and spread their legs. You're going to have a team of strong guys in the next room to hold your patient down if they don't listen to you, chemical restraints if you're by yourself and physical restraints if it's a prolonged exam. You need to learn empathy, compassion and kindness!”

“It's imperative that we operate in a drug free environment. That means checking every patient for contraband. As woman have become more and more creative with ways to hide drugs, our searches have unfortunately become more and more invasive.”

“After their initial exam they will be handcuffed and brought to you in an exam room. If it's a volentary exam so far they have been able to keep all their clothes on and nothing invasive has been done.”

“The first step is to get the woman out of there clothes. We do this right off the bat for two reasons. First we need the clothes removed eventually to examine them properly. The second - it establishes control. Woman who are naked are much more submissive and much less likely to argue with you.”

“Ask the woman to remove their clothes and either put on a gown or a uniform. Offer to leave the room to give them some privacy while they change. Most woman will choose the uniform because it is less revealing some will choose a gown. If they choose a uniform you will need to pat them down before proceeding with the exam. If they choose a gown this can be skipped.”

Standing in her scrubs she called a student up to assist her. “What's your name hun?”

“Robert”

“Okay Robert you're going to be demonstrating for me okay?”

Robert just nodded his head.

“Let's start by assuming that I'm a patient and have requested a uniform. Uniforms are basically the same as scrubs so this will be fairly accurate. You'll need to pat me down. To do that you want to get all the body limbs out of the way. You'll ask her to take a wide stance, then put her arms out to her sides.”

As she explained she demonstrated.

Stand behind me.

You'll start at the shoulders and in the squeezing motion work you're way out towards each hand. Robert followed her instructions but not to her liking.

“You're looking for knives, drugs, some of these things are small.” She took her hands and compressed his grip on her arm. “Squeeze! Make sure there's nothing but skin and bones”

“After you've completed the arms, look down the collar of the shirt.”

“Now slide your hands down my back using the palms of your hands”

Robert followed every instruction nervously.

“Run your hands down each of my sides.”

“Patting down the front of the body presents obvious problems. You have to make contact with my entire body without inappropriately touching my breasts. Work your way down from my neck until just before my nipple.”

Becoming more and more nervous Robert started to run his hands down Sammy's chest.

“Okay that's good now start at my waist line and move up until each hand meets the bottom of each of my breasts.”

“The legs are a little easier run both hands up my legs until you meet resistance from my body.”

Robert began but when he got most of the way up he looked up with baby eyes and asked sheepishly “Resistance?”

She again grabbed his hand and shoved it into her crotch.

“Keep going all the way up! Until you're sure there's nothing down there but me.”

Robert repeated on her other leg this time more confidently.

“Once we're certain that a patient is not carrying any dangerous weapons or drugs. We can proceede with the exam.”

“We're going to go through all variations of this exam. We'll start with the most common “IVC” or Involuntarily committed but cooperative. These are the kind of woman that don't want to be here but will let you do your evaluation without a struggle.

“We're going to start with a simple breast exam. Have the patient take a seat on the exam table and ask her to raise her top to expose her breasts”

Robert asked Sammy to lift her shirt up.

Sammy did as she was asked and Robert performed a routine breast exam. When he was done Sammy lowered her top and stood up.

“The next step is the pelvic exam. If they chose to wear a uniform then they will have to remove their bottoms, if they chose a gown then you can use a drape to keep them covered as they get into position. I'm obviously in a 'uniform' so I'm going to remove my pants. You can use a sheet to keep me covered while I get my feet into the stirrups.”

“When you're guiding her legs open don't use phrases like “Spread your legs” use phrases like let your legs fall apart.” Remember she's cooperating so there's no need to be forceful. Once she's in position uncover my genitals and perform a quick exam. You're not doing a yearly physical just a quick exam of the external genitalia to make sure theres no evidence of injury or rape when they arrive.” After you've completed your visual inspect you'll do a digital exam of my vagina, and of my rectum to make sure there is no contraband hidden.”

With Roberts finger buried in her lady parts she leaned forward looking down and asked him smiling “You find anything down there?”

Now extremely embarrassed he withdrew his finger and said “All clear”.

Sammy got up

“I'm going to get changed into a gown and we're going to repeat this exercise as a VC which is someone who voluntarily committed themselves.”

She pushed Robert out of the exam area and pulled the curtain closed.

She quickly changed into a patient gown open at the back and then pulled the curtain open. She called a new student Mark to come up and examine her. You're going to do the same thing as Robert.”

When Mark asked Sammy to lift her shirt he was surprised to see that she had put her bra back on.

He paused and looked at her puzzled

“What's the matter doctor?”

“Uhhh, you have you're bra on”

“You'd be surprised how often this happens. You tell patients to remove everything and put on a gown instead you'll find underwear and bra. Again because this is voluntary you don't want to force the issue, if this happens just use the opportunity to listen to the hart and lungs then leave asking her to remove the rest of her garments.”

Mark did as she asked then asked her to remove her bra. When he came back and attempted the exam again he was just about to begin her breast exam when she spoke up

“Mark do you notice anything strange?”

“No”

“My privates are exposed”

“Oh! I'm sorry I should give you a sheet”

“Yes you should.”

She recovered herself and waited for Mark to drape her with a sheet and ask her yet a third time to expose her breasts. Finally Mark was able to complete his exam.

The rest of the exam was nothing more than a very traditional pelvic exam.

When he was done Sammy took the class by surprise. Stood up, looked at Mark and said

“Here comes the fun part”

She through her gown on the floor.

The students in the room were shocked. Some of them were on the verge of laughter some of them were embarrassed, many were aroused.

“Mark what's on your mind”

“Well I've had a lot of training in my day and this would be the first time my teacher took off her clothes in front of the class.”

“It's your job to be professional! It's your job NOT to be judgmental! Not to be shocked! Control your facial expression. Control your emotions! This is what we call “INC” Involuntary Non-consensual exam. This is the meat and potatoes of this class. You're going to be examining woman that don't want you to see them naked much less touch them. You're going to be forcing them to sit still while you poke and prod at their breasts, as you force their legs apart in stirrups so you can shove your finger inside their vagina and their rectum.”

A smirk started to grow on Mark's face.

“You think this is funny!?!?”

“No”

“You think this is a game?!?”

Sammy turned around and grabbed her robe after she had put it on she turned to Mark

“Mark have you ever had anything up your butt?”

“what?”

“Have you ever had someone forcefully insert something up your rectum”

“No”

“Take off your pants!”

“What?...No!”

“Do you want to pass this course? I told you when you first walked in the door this course take empathy. Clearly you have a problem being sympathetic to a poor woman who's naked and under your control. You want me to sign off certifying you for these exams? Take. Off. Your. Pants.”

Mark slowly undid his belt and removed his pants. Standing there only in his tight white underwear.

“Keep going”

Mark was now standing naked from the waist down. Sammy asked him to bed over the table. He did as he was instructed. Sammy then inserted her finger up his rear. Mark recoiled and tried to escape her finger but the exam table was in his way.

“This is what it feels like Mark. This is what it feels like to be helpless on a table with your most intimate parts exposed and have a complete stranger do things to you not even your lovers have the privilege of do you understand what I'm telling you?

“yes!” he cried.

She removed her finger and discarded the glove. Go sit down she said.

She called another student up. Devin,

She removed her robe and began teaching again

“This is how INC exams will be presented to you. Most INC exams are due to suicide attempts. Law enforcement will have removed the clothes when the patient is admitted.”

Sammy began to demonstrate the different ways woman covering themselves.

“You'll see a lot of this”

(One arm covering both breasts one hand covering the genitals)

“Some woman don't care about covering their breasts but will cover their genitals.

(Both hands over the genitals)

“Occasionally you'll see a woman who is more cold then anything typically she'll cover one breast with each arm curling up like this”

“Whatever her reaction to having her clothes taken away you can't do an assessment. Arms need to be straight out on the side.” Breasts fully exposed, genitals in full view. “

Sammy demonstrated the proper posture for a patient.

“After she takes a seat on the exam table you can cover her up with a drape. Only expose the parts of her body you need to.”

“When you start the breast exam most woman won't offer too much resistance, if they do you can resort to either physical restraints or chemical sedation. Usually simply reminding a woman that if she doesn't consent to the exam 4-5 guys will come in and hold her down and then you'll do the exam anyway will end all discussion”

Sammy pulled the sheet to cover her nipples. Her breasts were still mostly exposed.

“Before we move on to the next part Try to put yourself in her shoes. You have taken all her clohtes away, all her dignity, and privacy. You have shown her that she no longer has control over simple things like her modesty, now you are going to force this poor girl to spread her legs, have you look at her most intimate parts, and stick your fingers in places she hasn't let anyone else.”

She gave the nod to continue.

Devin helped her feet into the stirrups. And exposed her genitals. As he continued his exam she continued to talk.

“Try to picture in your head what the patient is thinking. Here I am all alone, I've been stripped, I've been looked at. Now my legs are spread and some guy who I don't know has his fingers inside of me. If I try to fight back, they'll sedate me, or restrain me. It's as close to rape as we can get, a man is inside of me and I can't get him out. I'm helpless. He's in control and there's nothing I can do about it”
