As a child growing up in the 50’s and 60’s, if I was told to go to my parents bedroom and remove my pants and underpants, it meant that I was going to get a rectal temp taken, or an enema, or possibly a spanking. Sometimes it could even be a combination of the three. In the doctor’s office, it meant that I was definitely getting a rectal temp and possibly an injection. As a kid I didn’t mind getting rectal temps, but anything else scared me. As an adult I love them all.

I would agree with @Kiri, I like it matter of fact. No please, just a firm command. I think it really adds to the scene if that is the case whether it is a role play or a real doctor situation. It also helps if there is a bit of delay between the command and the doctor's action with that somewhat awkward silence in between. It gives you plenty of time to think about what is about to happen.
