In terms of duration.... I've had really good exams that may take 90 minutes, but I've also had good ones that lasted 30. Most have probably been somewhere in between. It depends on the play doc, and of course whether you include more time consuming procedures.. Less experienced doctors may not be up for the longer exam. And anything longer than 60 min or so starts to get a bit long for me.
As for the exam...
I like to start with a discussion of medical history and lifestyle questions with a few embarrassing questions tossed in.
Then I like to be asked to get vital signs... pulse, bp, and temperature. The latter is best done rectally with the doctor asking me to undo my pants and roll over on to my stomach. Maybe this is required for office policy or its observed that I've been drinking a cold or hot beverage. Then eye/ear/nose/throat check.
Then I am asked to get partially undressed (I like undressing gradually and ending up nude instead of wearing a gown) and being palpated and asuscitated. Reflex check, skin check, range of motion, etc. when the doctor is palpating my abdomen, it's important that they slip a hand under my waistband to feel around.
Then the underwear is slipped down for a genital exam. Inspection of penis and testicles. Check for hernia. Sterile swab test is ok.
Then the doctor puts me on all 4s to inspect my anal area and perform DRE and prostate check. They may determine that an enema is needed, which can be a fleet or bag. That is followed by further digital examination and then perhaps the use of an anoscope for a peek inside. I've also had a play doc use a larger proctoscope once.
Maybe medicine is required to be administered via suppository.
And we leave it at that or perhaps collect a semen sample, ideally with prostate stimulation.
I don't do anything with catheters, needles, or sounds. Also not really into photography.
I like a range of scenarios from pediatric or high school physical to employment or general adult wellness visit.
