In my case anyway, i would swap the word "fear" for "embarrassment" For me, "embarrassment becomes fetish"
Like i've always been afraid / not fond of needles, whether it's shots incoming, or blood going. That has not changed since childhood.
Now, let's consider embarrassment in regards to medfet, It was always embarrassing for me as a child to have to obey doctor's orders to "stand up & drop (my) pants" & be exposed & subjected to being seen & touched . It's like "I want to see you & touch you where no one else sees & goes, and you don't have a choice in the matter because you were brought here for this * this IS gonna happen" .Nothing to look forward to. Something to be awkward and embarrassed about - someone examining your privates.
Fast forward to adulthood & a change in procedure. You understand & accept the doctor's role & have gained the maturity to not mind this part of the physical. Nothing to get you excited/thrilled for, but nothing to really feel awkward or embarrassed about by this time. Only now, you're lying down while the doctor lowers the undies himself for a brief few seconds, & your hernia check is over so quickly, you go back to work & don't even dwell on it.
The subsequent years come & you know it'll be the same thing & it doesn't faze you at all...or does it ?
You have no attraction to the doctor because you're not into men. But you stare at the ceiling anyway, so despite the small talk dialogue, it may as well be anyone with a steth auscultating & palpating your abdomen. And then, you feel the waistband of your undies grabbed, followed by the feeling of open exposure. Yeah, you'd like it if your girlfriend grabbed a hold of your panties & pulled them down to play.You may start to think about the act of your panties being pulled down afterward & it may begin to be a point of arousal.
The years go on & though you never get erect during this procedure, you begin reflecting on it afterwards. Lying down as a vulnerable patient, subject to anything the doc chooses to do, anywhere the doctor choose to go. Eventually, he begins to do something extra.
He's completed the hernia check & instead of puling your undies back up, you feel ungloved fingers slide around your scrotum & you like to have it touched this way, as you compare it to "caressing".
The next year, the balls are gently squeezed one at a time. The doc never said "Now i'm going to check your testicles for lumps". He just went ahead & did it. You were lying there staring at the ceiling, and all of a sudden, fingers were manipulating your balls & you enjoyed it. In fact during 1 of those times, he started to guide your back with his hand to help you sit up when the exam was over. You couldn't just spring up like usual. What was that about.......
Similarly embarrassment & awkwardness became fetish with wearing jockstraps, although i've still never been seen wearing one. There may be no substitute for how a jockstrap grips & cradles.both my ass & genitalia.
