Rain is correct. The generally held rule of e-stim is never above the waist. The only exception really is with bi-polar clamps as you first described. This ensures the current path is extremely short and does not traverse the heart. The setup you described of nipple to clit would be a total no go from a safety perspective. E-stim is awesome, just be sure you play safe.
