College Physical Exam Forms

It seems to be the same form; excellent because of its conciseness.

This is a great form, indeed. I would be a bit unnerved at 18 to have to bring it to my ped, but now… great form!

I thought people might be interested to see a bit of a college physical exam form which was filled in about me from a few years back. I’ve never seen people posting their own medical forms on here but I’d like to see if anyone else wants to share:

Looks like a lot of 17 and 18 year olds in Illinois will be getting their first rectal exams.

My first rectal exam was during my college physical exam. My dad took me to his doctor who was a former Army doctor and was VERY thorough. I do not remember a form, however, but there might have been. But I do remember my first rectal exam just shy of my 18th birthday!

When I went to college I had to take an admission physical. I had the option of going to my own doctor or University Health Services.I chose my own doctor becaus we had a very good doctor/patient relationship. Although it was not on the form my doctor did order lab tests (blood and urine) due to some minor health issues.

I do remember dad taking me to the pediatrician for my college physical back the. Would be my last visit there then to a regular doctor after that. Dad was in the room I was stripped to briefs and socks. As they talked about other things doctor was performing the exam. Very THROUGH briefs came off rectal temperature then genital exam and of course he said turn around and bend over the table. He spread me and then i felt a finger slide in my rectum. First DRE for me! After that he said I can get dressed wished me luck in the future.

I think that a form is just that and it will be up to the doctor to determine what is to be done unless the items listed are mandated to be checked. Forms often are generic and cover a variety of ages. Still, seeing the items listed would make one think just how thorough an examination could be. I suspect that it is pretty common for a patient to be unaware of the detailed nature of the form. If true, it has to make for a more compelling examination.

Your doctor will see right through your ruse unless of course you are an athlete attending that college. Don't be foolish.

I detest these invasive questions. The schools are entitled for an opinion from a medical professional that the student is healthy enough to safely participate in the expected activities. Beyond that it is none of their fucking business.

@Ghost5 That form isn't even sufficiently thorough, brother. Genitals part is mentioned just in the first page - to be filled out by the students themselves. There is no indication the provided info will be confirmed medically later. An 'invasive' genital exam, especially just for the male, should be a section in itself. Its absence surprise me as I thought it'd automatically be a part, whether big or small, of such sports physicals.

Prostate exam for college... WOW

Yeah I'm trying to imagine what went through the minds of these young men as they completed the top part of the form as required ... and then looked at the checklist.

In 2013, hernia exams likely were expected at sports participation physicals. But "scrotum," "penis," "prostate," "rectal" ... wow. I wonder how many student-athletes became part of the medfet community because of these exams.

Things were no "better" for the young women. Arguably worse.

Just because it says prostate with a line _____ doesn’t mean it needs to be done.

From my experience, when I was in college, fields like that got a lot of attention just like they are in the forum. And the doc wrote “deferred” lol.

Nothing shocking at all. This is a standard run of the mill athletic physical.

@matty96 what fields do you find shocking? Looks pretty straight forward to me.
