I had this happen a couple of times for work physicals. When I was in undergrade during the summer vacation I worked as a "tester" for a lawnmower manufacturer. I had to test the new models by mowing parks, sports fields etc....I was offered the job and needed to get a company physical. The company had its own doctor. I was called back to the room, had the BP and height and weight checked and then the nurse says, "everything off except your underwear. The doctor comes in and starts with the basics and then has me "duck walk", stand on one leg and then run in place for a minute. Has me bend at the knees with my arms stretched out.Later around 30 or so, I had an employments physical same type of set up company had their own medical staff. Nurse did the same as above, waited for the doctor in my underwear. He asked for the same type of tests and additionally had me do jumping jacks. I was still in my underwear for both sessions.
For a real exam, i vaguely remember having to show some flexibility specifically touching my toes but not anything else. In 7th and 8th grade, there were the various Scoliosis checks shirtless and bent over. I haven't (yet?) been subjected to a real "stress test", but imagine at some point that i may have to go on a treadmill or bike or something.I have had some non-medical physical fitness/evaluation type of exams where there has been a lot of trying to demonstrate flexibility (like can you really touch your toes? 😃)In Play exams? Of course, can imagine all sorts of fun that can (and should?) happen in this space, all naked of course. 😃
Doing squats, hops, standing on one foot and various stretches aren't uncommon. I had one physical where they hooked me up to a heart rate monitor and had me do various walks, stretches, lifts, etc.
Only thing I remember is doing the duck walk in front of the pediatrician. Always awkward because it was always right after the testicular exam so ending the exam with something also embarassing.
During play exams, my doctor will have me to squats, sit-ups, push-ups and jumping jacks while naked.

I don't know why, but that whole part of pediatric and teenage physicals is hot to me. Duck walk, walk back and forth, even up and down steps sometimes.
When I see my doctor, I usually never remove more than my coat in the winter. This holds true for my PCP, Primary Care Physician, and my nephrologist.I actually bare more skin for a blood draw.
I've never had this done to me (honestly I don't think it's really a thing in England) but as if to compensate I've listened to my own heartbeat and breathing after exercise many times. I've often wondered why a test of someone's cardiovascular system under pressure wasn't part of a medical check-up, but as I guess I'd say perhaps it is for elite/professional athletes but not for most of the rest of us.
A sports gym I worked out at required a doctors release before I could sign up with a trainer. The doc ordered a stress ECG. Had a couple others in the Navy. If I asked, pretty sure my current doc would order one.
Yes for a pre employment physical I had to do some squats then bend over and touch my toes in one of those paper gowns that tie around the waist. The doctor commented on how flexible I am and asked if I was a dancer.
My daughter recently had her pre-participation exam for school sports. Her new doctor made her do the duck walk. She told her it was on the form so she had better do it. This was a first for her. Before anyone asks, no, I wasn't in the room with her at any point. My daughter told my wife and I about it when she got home. She thought it was weird and funny.
I’ve never had this in a real exam, but in online play exams I have done squats, used dumbbells, done some Pilates-type moves, and been listened to while working out on an exercise bike. Occasionally I have done a step test while examiner listens to evaluate fitness, but my recovery is always affected by my adrenaline over being stethed so it’s definitely not an accurate result! Being listened to while exercising does make it more enjoyable in a play setting for sure.
Now that certain elements are part of longevity, I've had some at my most recent. The Dr had me hang from a bar (to test grip strength), different one and two-leg poses to test balance, and hold a plank. All fully dressed, well I might have had my shoes off and been in socks.
A must, naked during the (play) examination, two or three sessions during the examination with BP and temperature checks.
When I had my nandatory physical exam where the military determines whether you're fit or unfit for conscription, I had to do several squats with my shirt removed for having an ECG. At another station they also tested my strength while I was fully clothed and the 2nd day the doctor had me bend forward and touch the toes in briefs only.
I’ve had to do several sets of 10 squats each being hooked up to a ekg machine, last physical did 4 sets of them and the doctor said let’s have you jog in place for a bit, while he was watching monitor and listening to me, then said let’s take a rest and I’m going to have you jog in place at a brisker pace. After about 5 minutes at a brisk pace he said if you can keep going while listening very closely then said try a little faster then said let’s stop and rest, I need you to repeat that again at the faster pace. Asked if he hear or sees something he said I want to get a better picture of your heart under a work load
I've never had this done to me (honestly I don't think it's really a thing in England) but as if to compensate I've listened to my own heartbeat and breathing after exercise many times.I'm fortunate that it was a thing in Scotland where we were listened to after exercising. I'm the same where I enjoy listening to my heartbeat and breathing sounds after working out at home. It's also a thing in play exams where my partner will get me to do star jumps or press ups, while stripped down to my boxers, and then listen to me after, telling me to push faster she wants to get my heart rate up.
I've never had to do it in a real exam. In play exams, I've had patients do some jumping jacks and squats while in their underwear or naked, and listened to their hearts and lungs as they recovered. They weren't very fit, and it took a while for their heart rates to come back down.
