I love exam role play also. When I'm in the doctors role I'll begin by asking her to strip down to her underwear and bra, and start by placing the cold stethoscope on her back and asking her to take deep breaths in and out. I'll also ask her to say 99 everytime I place the stethoscope as I go round again. I'll then listen to the front and repeat the process. Next, I listen on the back again to listen to her heart in several places and move onto the front listening to her chest and removing her bra, so she's now only in her underwear and nearly nude as I listen on her boobs. I'll then ask her to lie down and palpate her tummy before listening in all four quadrants and to the pulse above her bellybutton. Finally I'll listen to her femoral pulse.

When I'm in the patient role, it's the process in reverse. She'll be fully clothed and I'll be striped down to my boxer briefs as she goes all over me with a cold stethoscope on my back, chest and tum before putting it down my boxers, brushing against my dick in the process to listen to my femoral pulse.

I really like jambox's description of an examination. Having the patient strip to the waist and sit on the edge of the exam table while I carefully auscultate all the heart areas is my favourite fantasy.

My response would be similar to Scotty95's. I like to explain what I'm doing to the patient before I do it. If a woman's breasts are B cup or larger, it is necessary to lift the breasts to listen to the lower lobes of her lungs and to get a good listen to the mitral valve at the apex of her heart. I don't just put my hand on her breast without warning. I'll say, “I need to lift your left breast a little to get a good listen to your mitral valve.” I have also found it is generally easier to get a good listen to a woman's heart with her lying on her back and propped up on pillows so that her upper body is slightly elevated. I also like to have her lie on her left side for another listen at mitral.
