In the past, my regular doctor would say, "Now, if you'd stand up and lower your shorts for me, please..." But today's exam was by a different doctor, and he had a very different approach.
After the BP reading and routine questions, he said, "OK, stand, and take your clothes off." I was a little surprised, given that it's usually shirt off, and then pants open partially for palpating. But I got took off my shirt and my shoes, and then unzipped and stepped out of my slacks. As my fingers went to the elastic of my boxers, I said "Everything?"
He said "You can keep your underpants on."
On the exam table, after auscultation, I was told to lie on my back, and he began palpating my abdomen... very thoroughly and firmly, too.
Without his saying anything, his fingertips reached inside my underwear to extend his palpating to my lower abdomen. Then he pulled the elastic out slightly with his left hand and tucked his right hand entirely inside: cupping my penis and scrotum inside the fabric. After briefly pressing my penis between his fingertips and and moving it to each side, he began stroking through the folds of my nutsack and probing and rotating my testes, each in turn.
He only pulled the crotch of my boxers all the way down briefly, for a look... almost all of the genital exam was by touch, inside my underwear.
I had never had a doctor combine palpating with a genital exam in this way, before... It was always palpation with my pants mostly in place, though unzipped and open and sometimes slightly lowered (to allow the palpation into my lower abdomen), and then being asked to stand and pull down my underwear to bare my genitals entirely.
