H & P / Progress Notes From Real Doctor

Has anyone obtained copies of their chart notes taken from the gynecologist? Would be interested to hear what is written and detailed in the physical exam and assessment sections for a healthy patient receiving her annual.

I like to imagine an older male doctor with fewer patients (such as my wife's) taking time to make detailed notes about his exam findings. I am aware a very busy female gyn likely dictates as much as possible through concise short hand in their SOAP notes.

Being an attorney who has filed medical malpractice cases, I have seen thousands and thousands of pages of medical records, including records of gynecological examinations and I can tell you that barring some surgical procedure, they are mainly check the box forms with maybe a sentence or two of handwritten scribble. Not much else, unless something is out of the ordinary.

Makes sense. Check boxes are much faster. I did know a Dr. in another specialty who kept all his notes on plain notebook paper, which I had never seen before. I thought maybe there was some older male gyn like that who did not have a great deal of patients and didn't want to invest in EMR or pre-made print outs.
