I think El Guapo has the right idea, no need to involve others besides the patient, his consenting friend.
I strongly disagree with anyone saying it's no big deal about including the doctor or others. And, the shoe salesperson analogy doesn't work. The shoe salesperson doesn't ask the customer to disrobe and expose themself, nor will the salesperson be examining the customer in any way. The expectations for a doctor to protect privacy, and just the level of professionalism is hugely different.
And, any doctor, or shoe salesperson probably, would likely be very concerned about being included in photos or video which may appear on a porn site, or anywhere, even if their identity isn't clearly revealed. Just the rumor that such a thing happened in their place of business, with them present, could be very problematic. Thinking otherwise just isn't dealing with reality.
I'd say there would most certainly be damage done by such a thing happening, but the greatest damage would be to whoever participated in taking the images. In addition to losing their doctor, and possibly having trouble finding another doctor willing to have them as a patient, their own anonymity would be shot to hell in any lawsuit or legal charges being made, regardless of the outcome of such legal action. Obviously, the patient in this case, El Guapo's friend, is concerned about her anonymity.
