Actually, twice. In my 20's my PCP referred me to a gastroenterologist for investigation of my chronic constipation problems. The nurse came into the examination room, handed me a container and asked me to go try to obtain a stool sample. I was unsuccessful and returned to the room with an empty container. I figured they would just let me try to collect the sample later at home. However, when she returned and I told her that I couldn't go, she told me that she would give me an enema to obtain the sample. She led me down the hall to a procedure room and told me to remove my clothes from the waist down and don a paper examination gown which I should leave open in the back. When she returned, she had me kneel on the end of what I soon learned was a proctologic examination table. Once in position, she tilted the table up and folded it, putting me in a jack-knife position with my butt higher than the rest of my body. She retrieved a plastic 1500 ml enema bucket from under a counter and filled it to the top with warm water from a sink.
The enema was like none I had before. First, she did a digital rectal exam with gloved, lubricated finger. This was the first time that I had been probed like that in preparation for an enema. Then I felt her push the tube up inside of me. She opened the clamp and I could feel the water going in. It seemed to be running in deep because of the position I was in and how high she was holding the bucket. I also felt her advancing the tube deeper into my colon as the water flowed. I had never felt anything like that before. She kept advancing the tube until the bucket emptied. That really enhanced the sensation and I got hard . She apparently did not notice because of the position that I was in. As she withdrew the tube, it felt like a snake was coming out of me. It also felt very intense as the tube rubbed across my prostate. For a moment, I was afraid that I might ejaculate. She must have had the tube a good 18 inches inside of me, giving me my first colon tube enema. As soon as she had the tube extracted, she sent me off the bathroom that was adjacent to the procedure room, admonishing me not to flush until she was able to inspect the results.
After about five minutes, the nurse knocked on the bathroom door, asking if I was done. I had experienced an initial evacuation, but knew there was more enema up inside of me that was going to have to come out. I told her that I wasn't finished, but she insisted that she needed to see how I had done and provide me with the container to get the stool sample. I was pretty embarrassed, but let her enter to take a look. She made the observation that I was constipated and probably not empty. She told me to flush and follow her down the hall to the examination room. I did so reluctantly because I realized that I had not gotten rid of all of the enema. As soon as she left the examination room, I had to go again and soon. I headed quickly down the hall to the patient bathroom where I exploded. When I got back to the examination room, the doctor and nurse were waiting for me. The doctor asked the nurse how the enema had gone and she told him that I was badly constipated and that the enema had only been marginally successful. She didn't know that I had evacuated a lot more during my second trip to the bathroom.
After examining me, the doctor decided that he wanted to perform an unscheduled sigmoidoscopy. He reasoned that we could do it during that visit since I was already prepped. He asked the nurse if she thought I needed another enema and she replied in the affirmative. He left the examination room and she led me back to the procedure room where we repeated the 1500 ml tapwater enema while I was face down and butt up on the proctologic table. Again, she pushed the plastic tube a good 18 inches into me as she filled my colon. She remarked that I was probably going to feel a whole lot better afterwards and I had to agree.
Again, she only gave me about five minutes in the bathroom to evacuate. She knocked on the door and told me that the doctor was waiting to perform my exam. Again, I did not feel empty, but left the bathroom to undergo the exam. I do remember the sigmoidoscopy as being very uncomfortable. This was in the early 80's and this clinic still used the rigid scope. The doctor also remarked that I still had quite a bit of fluid in me. Fortunately, he was able to see well enough to complete the exam.
The second time I got an enema at the doctor's office was about 10 years later. It was a double Fleet, given to me by a colon and rectal surgeon, again in preparation for an unscheduled sigmoidoscopy. The doctor himself gave the enemas. His nurse was not even in the room. I was already in position on a proctologic table. He had just finished with a short proctoscope exam and told me that he wanted to do a full sigmoidoscopy. At almost the same time, I felt an enema tip being pushed into me from behind. I realized it was a Fleet when I felt the cold, burning liquid being squirted in. As soon as he withdrew the tip, I felt another tip enter and more cold liquid. He had just given me a double Fleet. He told me to hold the enemas until I couldn't stand it anymore, and then hold them another 2 minutes. I had a painful and crampy expulsion, but did not get a great result. Apparently it was good enough, because he was able to advance the flex sig almost 24 inches into my colon.
A few years ago, I got tap water enemas at an outpatient endoscopy center because of inadequate colonoscopy preparation, but I guess that doesn't qualify as getting an enema at the doctor's office.
