I have looked at reports and studies in medical journals before I started because I wanted to be safe. I found the following:
Saline and PEG enemas were the safest, with little or no visible irritation or damage to the epithelium. There were also no reported serious issues other than mechanical damage, such as someone pushing a nozzle through the rectal wall.
Glycerin, soapsuds, Fleet and bisacodyl all showed some epithelium damage and irritation and/or inflammation in studies.
Glycerin didn't seem to have any major "horror stories" (i.e. case reports) where it caused big damage.
Soapsuds has some "horror stories"
Fleet has the most "horror stories", with renal damage, necrosis of tissues around injuries to the mucosa, and other nastiness. However it does have the "plus" of being an approved, standardized pharmaceutical.
As a result when I started I started with the safest solutions - water and salt/soda. After a while I realized that in most or all of the soapsuds "horror stories" there was a significant amount of stupidity involved - antibacterial soap, detergents, crazy strong concentrations. I also realized that the colonic epithelium regenerates itself really fast, and that unless you were doing a biopsy right away there wasn't any danger noted. As a result I now am fine using stronger solutions, including soapsuds. That said there still are all those cases reported, and if something were to happen you bet they'd be trotted out and waved in peoples faces ("see here where this person put a whole bottle of Hibiclens in 1.5L of water and took it then had severe chemical colitis? Why did you recommend the use of 9mL of castile soap here knowing that soap can cause severe problems?")
