One thing I have been struggling with is how to convey the fact that you are into medical fetishes to someone who probably isn't without looking like a weirdo. I must confess, I'm of the opinion that "living a lie" or a half truth is such more work than just being upfront and honest about what you wish when conversing with someone else. It makes things so much easier when you're not tip-toeing around the subject you really wish to address. My advice, just be up-front and ask. If someone wants to carry it further, the cards are on the table,and they will proceed. If not, you won't be wasting time somewhere that isn't going to pan-out.

Being up front is definitely the best tactic. That said, you should ease into it: Don't expect to play right after discussing the fetish. Give your partner some time to digest, then suggest something more mild and always stay aware of your partner's comfort level.

I do think this is something better explored in a secure relationship than with hookups (vanilla hookups, not hookups specifically for the fetish).

I'm on the other side of the same problem. I know about my partner's fetish (he doesn't know I found out) but don't know how to tell him it's ok with me.
