For the largest enemas, I've found a couple positions that work. One is the well-known knee-chest position, that allows the fluid to flow downhill at least part of the way. it is a bit hard on the back, however.Something quite counter-intuitive is to take it standing up! I think this position makes it easier to clench your bottom to hold it in. I have taken a number of 4 quart enemas with a retention nozzle while standing.Another position is lying on the right side. It takes some pressure for the water to flow up and over the descending colon, but then it just pools in the cecum, and your right side can spill out over your hip bone. I've taken some really big enemas in that position, too.Jon
As I've written before, I wind up in at least three or four positions during a large enema depending on where the water is located and where I want it to go. However, after it's all in, say 3.5-4.5 quarts, I wind up either on my 1) right side down so as much water as possible stays as distal as possible and doesn't tend to want you make me push it out down low, or 2) (usually) on my back which I find the most comfortable for holding big enemas. I've been so comfortable with 4 quarts on my back that I not infrequently fall asleep holding 4 quarts with a latex DBC inflated in my bottom holding things in. (Yes, I'm a real person, doctor actually, who enjoys large long retention enemas because my over sensitive anus, alas, doesn't like multiple sequential fills and empties.) We'll see what happens tonight during my first (annual?) New Years celebration enema when I'm expecting to hold about 4 quarts of nice smooth saline until midnight (here, Central Standard Time, US), and maybe later if I wind up falling asleep again holding after midnight. This should be interesting. Definitely beats being drunk and watching largely uninteresting college football tomorrow. Sitting on my JBL I can take about 4Q if I work up to it. I find I can take large volumes best on the JBL.
My largest ones have been standing up. Been able to take more than 5 liters (don't know how much more I really could have taken, but wasn't into pushing my luck.The laying on the right side is also another high volume position.
Taking a large volume enema is always fun. I can take more water lying on my back in bed with several towels underneath. also a nice position is the shower stall sitting on a potty chair with the pot removed. hang the bag on the shower head lean back slightly and let the flow begin . also its nice to have the shower shot squirting hot water on your cods. this can make for a very large enema enjoyed.
I like to do 3 and 1/2 quarts on my back in bed with the bags hanging in front of me. I found that I get bigger volumes if I use an inline pump to enable more water to get higher up my colon. I also love to see my belly inflate in this position. I usually use a teaspoon of salt per quart of body temperature water. this allows for a thoroughly clean expulsion with no water Hang-Ups.
My sub is more comfortable laying on our bed, with me beside him, to take his most which is 2 1/2 to 3 qts. The bath tub is too much moving to get out so he can take as much comfortably.
Nurses in the “old days” would give the 3H enema with a patient in the knee-chest position, the most effective way for an enema solution to fill the colon.It may be the ultimate embarrassment to the patient, but it’s a time-tested position that works when giving a large volume enema.
I prefer the knee/chest position. I find it gives a better fill and if my caregiver is a bit free spirited will sort of make the delivery of the enema erotic as she does some “ jostling” of my position let’s say and can firm things up too!
Once you are all cleaned up, I would suggest laying on your back with legs as far apart and as high as possible, your butt also raised.....it is the most relaxing position for me and the one I've received and given the biggest enemas, especially with a retention nozzle....an inline pump can be helpful to fill the hardest to reach corners. I also suggest to do a slow filling raising the pressure as you fill.... I got the most filling with a colonic nozzle and about 6 ft, of tube in, starting very high and slowly descending, but...some leaking occurs, so I always do that in the tub..... actually, also did that in a boat a few times, years ago.... Got some very good fillings by using a retention nozzle and changing positions...starting on left, flat, right and back again to left...lot easier if you are younger....

Up until then, we received enemas laying across our bed or someone's lap. That summer, when I received my first 3H enema, it was the first time I had to get in the knee chest position. The following day, she started us on a daily rinse enema for a week. Those enemas were handled by our teenage aunts, who at the time, were responsible for our evening bath. When I want to take a large volume of liquid I start my enema laying on my back, then lay on my left side, and then finish the enema on all fours, head down butt up high.
I have tried all (well, that I can think of) positions. Occasionally I do knee/chest, but overwhelmingly I find 3 to 3 1/2 litres on left side, left arm behind me. Relatively slow… 2 or 3 minutes is best.
I do take large volumes, 4qts or to be precise 4 litres what is 4.2qts. My large bag came with me from Europe.I have never tried taking enemas lying on a side. Lying on my back, I can take up to 3qts, anything more get very uncomfortable. The larger volumes I take beginning in chest-knee position, and while enema progresses, I move to all 4s, so my torso is horizontal, then some times go back to chest-knee. As well, number of times I took 4qts standing leaning forward, with torso at about 45 degrees. With balloon nozzle this is actually quite easy.
My largest was 6 qts taken on my back in the bath tub with a 4 qt bag at maximum height. I filled it back up 1/2 way with soap again wide open flow and it was going in slow then i felt like a bubble or something in my intestines and the flow went so fast and the bag emptied.
I moved from laying on my back to laying on my side for 2 quarts, then finished the second bag on hands and knees.
On my back, as that allows the easiest and largest abdominal expansion. On my back I can easily take a gallon, any other position, much less.
Hi everyoneI still enjoy a large fill if I'm completely empty first, I've a large walk in shower with a bench, bag hooks and rails. Reclining on the bench or leaning on a rail is the best for me taking a large fill, 4 qts is still my fav goto large fill.BJ
For me, the best position for taking a high enema is the knee-chest position with the rectal tube inserted deeply.

Our grandmothers house had a big downstairs bathroom with a large dual head walk-in shower. On normal evenings, the girls put all of us in the shower together, then got in to bathe us and keep order. However, on evenings when the girls gave us enemas, like us, they were both naked. Being, the normal routine, was for them to get in the shower to bathe us and themselves.In the beginning, we received enemas laying naked across the girl's lap. When I was around nine, they switched us to the knee chest position. And once it became routine, the girls occasionally, had us get in the shower for enemas. Usually, we bend over a shower bench or got in the knee chest position. However, being only six years difference in age, the girls were more like siblings than aunts and liked to play around as much as we did. Hence, the large shower became a convenient place to play around when we tried different enema positions. We didn’t have to worry about possibly flooding the bathroom floor. Consequently, my siblings and I have a lot of fond memories growing up, playing around with the girls in the shower during bath time.
Usually knee chest in bathtub for my daily 3 quart coffee, but largest was on my back with legs bent did 7 qts.
