Your very first medical fantasy

One day thinking about my own early fantasies I became curious whether people still remember what was their 'very first' fantasy back in day (whenever the beginnings of medfet may have happened to you..) and whether you still use that fantasy today in some shape or form?

My own 'first' is pretty easy to tell, as I actually never stopped thinking about it. Naturally have discovered a ton of new fantasies since the early days, but the very first ones (with only minor modifications if any) still hold today..

For me it is definitely 're-living' of my childhood pediatrician exams with some twists. Perhaps, the very earliest experience I remember that I later recognized as "erotic" happened when I was only three or four years old. My first real fantasy stemmed from an experience I had at a pediatricians office around 11, one that I managed to get out of... but now wish I wouldn't have. I really don't remember my first fantasy per se (I wish I did). I think my original fantasies, which I would t have known them as at the time, all came from a photo in a book I had.
