When I was a child, bathrooms stalls without doors were very common in public facilities. In fact, going to the park or the beach you would hardly ever see any doors on the stalls. It was the same with public schools, none of the schools I went to had doors on the stalls, When I was in first grade, our classroom had its own bathroom, and it was just two toilets and a sink, no partition or anything. The first time I had to pee in this class, I went during a break and was shocked to see this arrangement. I tried to pee standing up, but I couldn't with someone next to me. There was a line outside, so I tried sitting down. No luck. I tried to wait til the day was over, but felt like I had to burst and ended up going during class when no one else was in there. I actually asked my mom what was wrong with me when I got home, because I wasn't really embarrassed about being seen, I just could not go no matter how hard I tried. She said I was just "stool shy". For the rest of my school years, I seldom went to the bathroom at school. I would hold it until I got home.
I also encountered a hospital children's ward boy's room with the same arrangement as the first grade bathroom; no partitions, just two toilets and a sink...and you had to be taken there by an adult. I stayed at this hospital when I got my tonsils out, so my solution was to try and wait until night time to go. The first time, the nurse came in with me and stood there watching, and again I couldn't go. I had to ask her to stand outside so I could go.
I have seen some strange bathroom arrangements with open stalls. The weirdest was at a park in Las Vegas, there was a door on both sides of the building and when you walked in either door, the first thing you saw was an open toilet. Then a brick partition, and behind the partition was the urinal, with the sinks in the middle. I wondered who would design a bathroom where the toilets are the most open feature and the urinals and sinks are behind a partition. Obviously, I did not use this facility.
