I recommend massaging the vaginal area with this sort of vibrator before inserting the speculum. Apply a small amount of water-based lube so the knob of the vibrator slides easily up and down and all around the sensitive folds. Take your time and talk gently with your patient to determine the area that she likes stimulated. I have a Panabrator VII which I think is better than the more common Hitachi Magic Wand. The head is angled for easier reach, it feels more powerful and quieter, and it uses a thumb dial to adjust speed instead of having just two preset ones. These are getting difficult to find though, I think the product line has been discontinued for a long time already.
