At the pediatrician's (in the past):
The genital exam always came last. It was always done while standing. The doctor sat on a stool. My underpants were pulled down. Visual inspection of the genital area. The penis was held in hand, and the foreskin was retracted. A short squeeze of the glans and inspection of the urethral opening. The foreskin was pushed back, followed by a short palpation of the testicles.
At the urologist (today):
I lie on my back on the exam table, without trousers or underpants. Ultrasound of the bladder and abdominal cavity. Then the penile exam: retraction of the foreskin, inspection of the glans all around, squeezing the glans with thumb and index finger and inspecting the urethral opening, palpation of the penile shaft, and pushing the foreskin back. Then the testicular exam: the first testicle is palpated and gently squeezed between fingers and thumb, followed by the other testicle. Then both testicles are shortly palpated simultaneously, along with the spermatic cords. A short inspection of the perineal area by lifting the scrotum. Finally, a rectal exam and palpation of the prostate while lying on my side.
At the dermatologist (today):
Preventive screening while wearing only underpants. Inspection of all body parts from head to toe using a dermatoscope. While standing with my back to the doctor, I pull my underpants down halfway through the exam, and my buttocks are spread apart. Afterward, the underpants are pulled back up. At the end, while standing—with the doctor seated on a stool—I am asked to pull my underpants down and let them drop. Inspection of the genital area. Lifting the penis, inspection of the skin on the underside of the penis, inspection of the scrotum, shifting the scrotum to the left and then to the right. A question regarding skin changes on the glans; the foreskin was not retracted.
