interesting question, here's my view: being my fascination about medicine and Women's Health coming out from the very real thing, my perfect patient would be the one who really feels herself as a real patient, undergoing a real medical exam.
The perfect situation is - to me - the one where the two are not simulating, but "living" their respective roles. Not sure if I can explain it the right way, but for me this is the key point. I do my best to second this view, from improving my medical knowledge, to the room setting (which is close to a classy doctor's office, not a dungeon), from the time I spend interviewing the patient, to the credibility of the procedures.
I had a patient a couple of weeks ago, it was her first examination with me, and the first with a playdoc. She was a bit nervous and a bit reluctant to undergo some parts of the exam, mostly because of embarassement. In the end, she agreed to do 90% of what I defined necessary, and the exam was over. We exchanged a couple of emails the day after, and she wrote me: "I never felt myself playing a role, and never felt you as a fake. It was like being at the doctor, with all its content of embarassing, intriguing, anxiety etc..."
This is for me a perfect situation.
Of course the final result of any setting involving two people mostly depends on what you can establish with the other one, which is built on compatibility and matching views.
Finding it out, always requires communication, the other special, exciting, intriguing ingredient.
